- The pre-solicitation synopsis issued on 13 April 2006 is modified to include the following: A contract will not be awarded to a firm or a subsidiary of a firm if it is determined that the government of a terrorist country has a significant interest in the firm or subsidiary of a firm or if the firm is determined to be under foreign ownership, control or influence of countries that support terrorism or are considered critical intelligence threats. Subcontractors identified in the proposal will be subject to the same eligibility determination. Interested prime and subcontractors must submit DSS Standard Form 328, Certificate Pertaining to Foreign Interests. The form may be downloaded at the website http://www.dss.mil/isec/sf328.pdf. It is highly recommended that Contractors submit SF328), prior to submission of the proposal in order to obtain preclearance. If the DSS Standard Form 328 is submitted prior to the proposal receipt date, the form shall be submitted to Ms. Ann Saki-Eli at ann.sakieli@navy.mil together with a statement as to the type of work to be performed under the contract (i.e., prime contractor ? clearing/grubbing, foundation, excavation and fill; subcontractor A, interior and exterior lighting; subcontractor B, wet-pipe fire suppression sprinklers). Otherwise, the DSS Standard Form 328 shall be submitted with the proposal in accordance with the Request for Proposal. (Please note that the SF328 is for the purposes of determining contractor responsibility. This is NOT a classified contract and a classified facility security clearance is not required). The work will also include two pre-priced Options for 12-month elevator and escalator routine warranty service in accordance with manufacturer?s warranty requirements. In accordance with FAR 52.217-7, Option for Increased Quantity?Separately Priced Line Item, the Government may exercise the option by written notice to the Contractor within three months after acceptance of the elevators and escalators. A pre-proposal conference will be on 7 July 2006, 8:00 a.m., HST at the HOC Okinawan Center, 94-587 Ukee Street, Waipahu, HI 96797, where questions will be answered relative to this Request for Proposal (RFP). Comments, constructive criticism, and RFP inconsistencies are solicited as well. ALL PROSPECTIVE OFFERORS ARE URGED TO ATTEND THIS CONFERENCE. Minutes of this meeting will be NOT be issued. The use of cameras, tape recorders, and other recording devices are prohibited during the pre-proposal conference. The pre-proposal site visit has been scheduled for 7 July 2006 immediately following the pre-proposal conference. The site visit will be conducted in accordance with FAR 52.236-27. This site visit is to provide Offerors with an opportunity to familiarize themselves with the jobsite and conditions to be encountered and is not for the purpose of answering questions. The use of cameras and camcorders are prohibited at the site visit. Offerors interested in attending the pre-proposal site visit must email the following information to Ms. Ann Saki-Eli at ann.sakieli@navy.mil on or before 30 June 2006: FIRM NAME, FIRM TELEPHONE NO., VISITOR'S NAME/TITLE, VISITOR'S SSN, VISITOR'S HOME ADDRESS, CITIZENSHIP (must be a U.S. citizen or naturalized U.S. citizen), DATE/PLACE OF BIRTH; ATTENDING PRE-PROPOSAL CONFERENCE: YES OR NO In addition, Offerors must submit a completed ?Hold Harmless? form no later than 30 June 2006. The ?Hold Harmless? form may be downloaded at www.esol.navfac.navy.mil. ?Hold Harmless? forms may be submitted electronically to ann.sakieli@navy.mil however, the form with the original signature must be mailed immediately to NAVFAC Pacific, Construction Contracts Branch (ACQ11), ATTN: A. Saki-Eli, 258 Makalapa Dr., Ste 100, Pearl Harbor, HI 96818-3134 so that it is received prior to the site visit to ensure the visitor will be allowed access to the site. If the electronic application information is received after 30 June 2006, the applicant WILL NOT be allowed to attend the site visit. The furnishing of the required information, including your Social Security Number, is voluntary. However, your failure to furnish all or part of the information request may result in the Government's denial of access to the jobsite. Any of the above information furnished by you and protected under the Privacy Act shall not be released unless permitted by law and/or you have consented to such release. All other information in the synopsis issued on 13 April 2006 remains unchanged.
