- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, FM4427-6145-0897, is issued as a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-09 and Defense DCN 20060616. The Federal Supply Class (FSC) is 6515. The associated Standard Industrial Classification (SIC) is 3841. The North American Industry Classification System is 334510 and the associated business size standard is 500 employees. If two (2) or more responsible and technically acceptable small businesses respond, this acquisition will be 100% small business set-aside; if not, it will be unrestricted. This request for quotation consists of the following: 1 Each, Destrusand 500 Device and Convenience, Part # 185_1100, Version URO Including: Destrusan 500 Basic Appliance, Use Guide, FDA Conformity letter, Final Check Report, Power Cord, Emergency Stop Switch, Patient Cable for Single Polar Probe, Universal Extension Cable, Box: 15 ea Electrotherapy-Catheter, Ch 8- For Med Research Only, Ch 10- For Med Research Only, Ch 12- For Med Research Only, Anal Probe Bipolar, Vaginal Prove Bipolar, Pkg. Adhesive Electrode, Reduction Adapter 4mm- 2mm, Connection Adapter4mm- 4mm, Catheter Adapter for Patient Cable Sanica, Peritrol Maes. System Device (vaginal), Or Equal. Delivery to Travis AFB shall be completed within 30 days after receipt of order. The following provisions and or clauses apply to this acquisition.
