- The VA National Acquisition Center will issue VA 797-06-RP-0064 for the Department of Veterans Affairs (VA), and Department of Defense (DOD), requirements for Glyburide tablets. BOP and IHS requirements will be forthcoming. Glyburide tablets have an estimated annual usage 51,887 bottles of the 1.25mg/100s and 287,195 bottles of the 2.5mg/100s; 21,202 bottles of the 5mg/100 and 372,150 of 5mg/500s OR 201,819 of 5mg/1000s. NOTE: Where there is a choice of package size as indicated by the word "OR", the offeror must submit a price on at least one of the two sizes indicated. Items must be awarded as an aggregate. Award will be made on the package size most advantageous to the Government. The estimated quantities listed above are for VA, DOD, IHS, and BOP. The contract period will be for one year plus four one-year option periods. The contract items will be distributed through the VA and DOD's respective Pharmaceutical Prime Vendors (PPV). The contracted items will be automatically substituted at the PPV; all other products will be locked out for VA only. Offerors must provide bar code labeling which must state the exact name (name that appears on label) by which the drug will be supplied and must have a unique NDC number. The first 5 numbers must identify the offeror. Bar Code Validation is the process of ensuring the data on a bar code label received from suppliers meet minimum requirements for font, readability, data integrity, and completeness. All bar codes must be in a linear format and conform to all UCC/EAN standards. This means the bar code can be any linear bar code symbiology, such as UCC/EAN-128, RSS, or UPC (if the UPC contains the NDC). This procurement is subject to Good Manufacturing Practices (GMPs). Bar code must provide the expiration date and lot number. Offerors must provide bar coded product that meets these minimum requirements. In addition, tablets must be compatible with automated dispensing units (Baxter ATC Canisters, Optifil, etc.). Glass bottles are not acceptable. All bottl es of 100s must have child resistant closure. All bottles must have a labeling surface with minimum dimensions of 2.25 inches in height and 5.0 inches in circumference. If the offeror is not a manufacturer, a signed agreement or letter of commitment between the offeror and the manufacturer must be submitted with the offer and remain in effect for the duration of the contract period including the option years.
