- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes ar e being requested and a written solicitation will not be issued. The solicitation number is W91YTV-06-T-8401. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-05. The North American Industry Classification System Code (NAICS) for this procurement is: 339112. This requirement is unrestricted. Blanchfield Army Community Hospital, Bldg. 650, Fort Campbell, Kentucky, has a requirement for the following: TWO SETS OF 4.5MM CANNULATED STD SCREW SET, 1/3MM THD, CAT. NO. 00-1147-001-45; UNIT PRICE $__________, TOTAL CLIN $__________. TWO SETS OF CANNULATED SCREW 4.5MM INSTRUMENTS, CAT. NO. 00-1147-000-45; UNIT PRICE $__________, TOTAL CLIN $__________. TWO SETS OF 4.5MM CANNULATED STD SCREW SET, FULL THD, CAT. NO. 00-1147-002-45. The delivery terms will be for all material to be delivered on or about 3 July 2006. Please include shipping and handling cost if applicable. The government anticipates awarding a single award for this requirement. To be delivered to: Blanchfield Army Community Hospital, USA MEDDAC, Medical Supply, 650B Joel Drive, Fort Campbell, KY 42223-5349.
