The intent of this solicitation is to provide ?Complete? training of the ?Return to Work Program? developed by New York University at Naval Medical Center Portsmouth. Vendors with superior past performance histories may be given 1st consideration. Section B - Supplies or Services and Prices ITEM NO SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 1 Lot Performance of "Return to Work" FFP Education Program / Class (See Section C for Details) FOB: Destination MILSTRIP: N0018306RQTC200 PURCHASE REQUEST NUMBER: N0018306RQTC200 NET AMT Section C - Descriptions and Specifications CLAUSES INCORPORATED BY FULL TEXT Statement of Work Selected vendor shall be required to perform total training of the ?Return to Work? program developed by New York University. The Government shall accept quotes on alternate programs, provided that alternate program is of equal or better technical value. Alternate programs must be fully established and approved by the American Medical Association, American College of Surgeons or Associations of American Physicians & Surgeons. The Government reserves the right to make final determination of ?equal or better? alternate programs. Selected vendor shall be certified / approved by the New York University Medical Center / New York University School of Medicine in the education of the ?Return to Work? program. All training shall be conducted on-site at Naval Medical Center Portsmouth during a mutually agreed upon time (between selected vendor & Government). All training shall be completed no later than 30 Sep 06. Selected vendor shall be required to provide all training materials including text books and course work. Training shall be conducted / supervised by personal with a minimum qualification of PHD and MD. Combined, the faculty must represent expertise in the following fields: Psychology, Rehabilitation, Orthopaedics, Physical Therapy and clinical research.. Minimum class duration shall be 24 hours. Classes shall be conducted over a continuous 4-day period. The Government shall supply classroom(s), general computer equipment. The estimated class size shall be 20-25 students. Upon the completion of training students are expected to have the knowledge to properly evaluate patients on their ability to ?return to work? and to train additional personal to evaluate patients for the ability to ?return to work?. All classes must qualify as ?continuing medical education credit? for medical personal. Topics to cover include as a minimum the following: Standardized clinical evaluation for low back pain Bio-Psychosocial Paradigm Reconditioning program Cognitive-Behavioral Therapy Liaison Workplace Use of Clinical Questionnaires Multidisciplinary teamwork in the management of low back pain Vendor?s price shall be inclusive of all cost including, travel, training material (Government personal are expected to retain all training materials including textbooks), labor and any required per diem.
