- Description: The Network 22 Logistics Office, 5901 East Seventh Street, Long Beach, CA 90822 has a request for Purchasing Skilled Home Nursing Care Services, which include Physical Therapy, Speech Therapy, Occupational Therapy, Infusion Therapy and Medical Social Services for the VA Southern Nevada Healthcare System beneficiaries. The counties covering are Nevada (Clark, Nye, Lincoln and Esmeralda), Arizona (Mohave County) and Utah (Washington County). This contract will be a one (1) base year, with four (4), one (1) year options. Issuance of the package will be the week of June 19, 2006, with a response date of July 17, 2006 @ 4:00 p.m. PDT. All responses for the package shall be in writing and faxed to 310-268-4759 or mailed to the Contracting Officer at the VA Greater Los Angeles Healthcare System, 11301 Wilshire Blvd., Bldg. 258, Room 210, Los Angeles, CA 90073.
