- The Bureau of Prisons is seeking quotes from sources having the ability to provide community based outpatient assessment and counseling services for male and female Federal offenders held under authority of United States Statutes located within the Eau Claire, Wisconsin area defined as follows: 3136 Craig Road, Eau Claire, Wisconsin or within the vicinity of Highway 37 North and West, Hamilton Avenue to the South and Stein Boulevard to the East of 3136 Craig Road and accessible to a local public bus transportation route, within two blocks that provides direct service to and from 3136 Craig Road, Eau Claire, Wisconsin area. Performance period under this contract will be for a one year base period to begin on September 1, 2006 with four one year option periods with service ending (if all option years are renewed) on August 31, 2011. The services required for this contract include Line Item 1: Base Period, Option Year One, Option Year Two, Option Year Three and Option Year Four, respectively. Base Year, Estimated Number of Inmates 10: (2) Intake Assessments; Individual Counseling Sessions (30 minute = 1 unit) 48 Individual Units x 10 inmates = 480 total units; Group Counseling Sessions (30 minute = 1 unit) 72 Units x 10 inmates = 720 total units; (1) Mental Health Intake Assessment and Report (1 unit per 2 inmates) 2 total units; (2) Medication Monitoring (1 unit per 2 inmates) 4 total units. Option Year One, Estimated Number of Inmates11: (2) Intake Assessments; Individual Counseling Sessions (30 minute = 1 unit) 48 Individual Units x11 inmates = 528 total units; Group Counseling Sessions (30 minute = 1 unit) 72 Units x 11 inmates = 792 total units; (1) Mental Health Intake Assessment and Report (1 units per 2 inmates) 2 total units; (2) Medication Monitoring (1 unit per 4 inmates) 4 total units. Option Year Two, Estimated Number of Inmates 12: (3) Intake Assessments; Individual Counseling Sessions (30 minute = 1 unit) 48 Individual Units x 12 inmates = 576 total units; Group Counseling Sessions (30 minute = 1 unit) 72 Units x 12 inmates = 864 total units; (1) Mental Health Intake Assessment and Report (1 unit per 2 inmate) 2 total units; (2) Medication Monitoring (1 unit per 2 inmates) 4 total units. Option Year Three, Estimated Number of Inmates 13: (4) Intake Assessments; Individual Counseling Sessions (30 minute = 1 unit) 48 Individual Units x 13 inmates = 624 total units; Group Counseling Sessions (30 minute = 1 unit) 72 Units x 13 inmates = 936 total units; (1) Mental Health Intake Assessment and Report (1 unit per 2 inmates) 2 total units; (2) Medication Monitoring (1 unit per 2 inmates) 4 total units. Option Year Four, Estimated Number of Inmates 14: (5) Intake Assessments; Individual Counseling Sessions (30 minute = 1 unit) 48 Individual Units x 14 inmates = 672 total units; Group Counseling Sessions (30 minute = 1 unit) 72 Units x 14 inmates = 1,008 total units; (1) Mental Health Intake Assessment and Report (1 unit per 2 inmate) 2 total units; (2) Medication Monitoring (1 unit per 2 inmates) 4 total units. The services described shall be in accordance with the Federal Bureau of Prisons Transitional Drug Abuse Treatment (TDAT) Statement of Work dated October 2005 and Wisconsin Out-Patient Drug Treatment License. The solicitation will be available on or after June 23, 2006 and will be distributed solely through the General Services Administration?s Federal Business Opportunities web site http://www.fedbizopps.gov. Hard copies of the solicitation document will not be available. The site provides downloading instructions.
