Exact quantities from that delivery order shall be oral or faxed to the contractor 2-3 times a week as necessary. Once an order is placed, the contractor shall deliver quantities ordered NLT 48 hours after confirmation of order. 4.1.4 Contractor shall track quantities delivered for reconciliation purposes. 5.0 ADMINISTRATIVE CONSIDERATIONS None 6.0 INSPECTION/ACCEPTANCE CRITERIA 6.1 Completed by Government 7.0 DATA REQUIREMENTS (REPORTS, TECHNICAL DATA, ETC) 7.1 N/A 8.0 PACKING, PACKAGING, AND MARKING REQUIREMENTS 8.1 See FAR Clauses herein this solicitation. 9.0 DELIVERY SCHEDULES/SPECIAL INSTRUCTIONS 9.1 The contractor shall make deliveries to the following address FOB DESTINATION on Tuesday and Thursday of each week between the hours of 0600 - 1400: Charette Health Care Center Nutrition Management Dept, Bldg. 2, 1st floor Naval Medical Center Attn: Perla Colgan 27 Effingham Street Portsmouth, VA 23708-2197 (757) 953-1003 All deliveries will be made to the 1st floor loading dock Tuesday and Thursday between 0600 - 1400 10.0 CONTRACT ADMINISTRATIVE FUNCTIONS None 11.0 OTHER DATA PERTINENT TO REQUIREMENT None 12.0 PAYMENT SCHEDULE / INVOICING 12.1 Contractor shall invoice no more than twice per month and shall attach a copy of all delivery tickets for quantities delivered during that period. 12.2 The contractor shall not exceed the dollar amount or quantity allowed on individual delivery orders without prior approval from the Ordering Officer. 12.3 Payment shall be made by government purchase card monthly OR payment shall be made quarterly. SPECIAL REQUIREMENTS CHEMICAL AND MICROBIOLOGICAL REQUIREMENTS: As defined in the Veterinary/Medical Wholesomeness Assurance Program for Fresh and Cultured Dairy Products and Frozen Desserts (AR-40-70/NAVSUPINST 4355.6/AFR 161-46/MCO 10110.44), the following chemical and microbiological requirements are included as a result of this solicitation for milk, milk products, and cultured products: (1) Chemical requirements. Products shall meet the chemical requirements for each specification cited in the contract on the date of award. (2) Microbiological requirements. Products shall meet microbiological requirements stated in Public Health Service Publication 229, Grade A Pasteurized Milk Ordnance, in effect on the date of award. In the event of conflict between these requirements and individual product specifications, the requirements of Public Health Service Publication 229 take precedence.
