Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This combined synopsis/solicitation is issued as a Request for Quote (RFQ) reference number W91243-06-T-0008. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-09. This procurement is being issued as a 100% Total Small Business Set-aside. This will be one time award for Dental Services from 21-23 July 2006. STATEMENT OF WORK FOR MOBILE DENTAL SERVICES 1.0 Scope. This Statement of Work (SOW) covers dental exams and treatment for the Soldier Readiness Process (SRP) for the 755th CM CO (SECFOR) to be completed on 21, 22 and 23 Jul 2006. This SOW identifies the purpose of the work to be performed by the contractor and provides a description of the work to be completed and any required outcomes. The contractor shall provide qualified personnel and material to perform the following services in accordance with this SOW. 2.0 Purpose. The purpose of this project is to complete required annual dental examination to assign a dental classification of the risk of having a non-combat dental emergency during deployment and provide dental treatment to soldiers identified as Den tal Class III and bring them to a Dental Class II classification. Soldiers classified as Dental Class III have a condition that may cause a potential dental emergency during deployment. Once the condition is corrected, the soldier will be reclassified to a Class I or Class II. Soldiers in the Dental class III or IV category are ineligible for deployment. Dental Readiness is fundamental to maintaining unit readiness and reducing non-combat dental casualties during deployment. 3.0 Reference. 3.1 AR 40-501 Standards of Medical Fitness 3.2 AR 40-35 Dental Readiness and Community Oral Health Prevention 3.3 Department of the Army Personnel Policy Guidance (PPG) 4.0 Services. The contractor shall provide all management, labor, material, equipment, certifications and supplies necessary to provide mobile dental services using dental vehicles. This service will be delivered at the Medical Command Clinic 21, 22 and 2 3 Jul 2006 at the Medical Command Clinic, 2411 W. Butler Ave, Lincoln, Nebraska. This service will provide mandated dental examinations and treatment for approximately 190 soldiers required to meet dental readiness and deployment standards to ensure missi on capability of the Nebraska Army National Guard. 4.1 Tasks 4.1.1 Dental Examinations. The Contractor shall conduct Dental Examinations in accordance with AR 40-501 and AR 40-35 to include: 2 bitewing radiographs for 190 personnel. Panograph x-rays shall be required for personnel with xrays greater than 10 years old or less than 10 years if the soldier has had significant dental work. The Contractor shall ensure that 75 soldiers are examined during an 8 hour day. Dental Exams will be conducted in the Nebraska National Guard Joint Medical Clinic using fixe d dental equipment to include three dental exam rooms with digital bitewing capability and digital panograph equipment. The contractor will carry their own insurance for damage or liability reasons. The contractor will ensure that all soldiers have a dent al panograph on file in their dental record. The contractor shall also ensure adherence to the requirements of the Occupational Safety and Health Administration (OSHA) and applicable Federal regulations. 4.1.2 Dental Treatment. The contractor shall provide dental treatment for all soldiers identified as Dental Class III to correct the condition required to be upgraded to a Dental Class II using the contractor provided Mobile Dental Vehicle to treat approximately 55-60 soldiers on the 22-23 July 2006. The contractor shall provide all staffing, supplies and equipment required to provide dental treatment for 55-60 soldiers. The contractor shall also ensure adherence to the requirements of the Occupat ional Safety and Health Administration (OSHA) and applicable Federal regulations. 4.1.3 Documentation. The contractor will ensure that all Soldiers have a complete dental record to include military dental record jacket, DA Form 5570 (Health Questionnaire envelope), privacy act statement, SF 603/603a and/or DD Form 2813, HIPPA noti ce of privacy practices, and supporting radiographs. The panographic radiograph will adequately represent the current oral condition of the Soldier. The contractor shall complete data entry of dental classification and panograph completion into the MEDPRO S/MODS system. 5.0 General Information. 5.1 Quality Control. The contractor shall adopt a quality assurance program that monitors all service activities and ensures the highest quality customizable dental services available. It is the responsibility of the ARNG to review and approve dental tr eatment plans before treatment is performed based on the guidelines provided by NGB. 5.2 Hours of Operation. The Nebraska National Guard Joint Medical Clinic will open at 0700 on 21, 22 and 23 July 2006. Soldiers will be processed from 0730 until completion each day. 5.3 Security Requirements. The base security main gate will be notified of team arrival. All contract employees will be required to show identification at the main gate upon arrival. 5.4 Reporting. The contractor shall provide a final report to include the number of soldiers screened, a by-name roster of soldiers identified as a Dental Class III and the status of the dental treatment.
