Description
Medical Supplies - Potential Sources Sought Military Hospitals: (Army, Navy, Air Force) Southwest Region DEEP VEIN THROMBOSIS (DVT) PROPHYLAXIS GARMENTS AND DEVICES ? Standardization Point of Contact: Cindy Ingrao, Clinical Consultant, Tri-Service Regional Business Office, Southwest Region, San Diego, CA. Telephone: (619) 532-8517. Potential sources sought to participate in the regional standardization of DVT Prophylaxis Garments and Devices. Southwest Region, a military integrated delivery network (IDN), comprised of Army, Navy and Air Force medical treatment centers in California are in the process of standardizing DVT Prophylaxis Garments and Devices. If your company currently holds or is in the process of obtaining a Distribution and Pricing Agreement (DAPA) with the Defense Supply Center-Philadelphia (DSCP) for DVT Prophylaxis Garments and Devices, your company is eligible to participate in the standardization process. If your company is not a current DAPA holder for this product line, you are still eligible to participate in the standardization process by contacting the Defense Supply Center, Philadelphia (DSCP), Ms. Kim Nichols at (215) 737-8740 or e-mail at Kim.Nichols@dla.mil to apply for a DAPA. Your company can be considered once DAPA paperwork is pending at DSCP. Please respond by Email: cingrao@nmcsd.med.navy.mil and provide your company name, address, point of contact, telephone, fax and E-mail address and answers to the below listed questions no later than July 19, 2006. For questions, please call (619) 532-8517. 1.Does your company manufacture a full line of intermittent pneumatic compression sleeves and devices for adults and pediatrics? 2.Are your sleeves available through the Prime Vendor, Owens & Minor and Cardinal? 3.What is your DAPA Number? 4.Under what brand(s) does your company manufacture or distribute your product? 5.Under what standards are your sleeves and devices manufactured? 6.What types of intermittent pneumatic compression sleeves and devices does your company offer? 7.Are your devices capable of sequential and circumferential compression? 8.What types of materials are your sleeves available in? 9.What features do your sleeves have that make them comfortable for the patient? 10.Do your devices provide pressure between 45-52mm Hg +/- 10%? 11.Do your devices provide intermittent pneumatic compression to the lower extremities and mimic the leg?s muscle action as if the patient were ambulatory? 12.Can your devices be used with only one sleeve? 13.What is the weight of your device/pump? 14.How is your device/pump attached to the patient bed? 15.Are your sleeves latex free? 16.Are your sleeves available in a variety of sizes for pediatrics and adults? If so, what sizes? 17.Does your company have a sleeve for feet only? 18.Do your devices have the capability of functioning on only one extremity? 19.How are your sleeves secured to the patient? 20.Can your sleeves be reprocessed and reused? 21.What is the length of your connecting tubing? 22.Do your devices/pumps have an alarm? In what circumstances does the alarm sound? 23.Do your devices/pumps have a lock out mechanism? 24.How does your company recommend the device/pump be cleaned? 25.Does your company have a conversion plan? 26.Does your company have a maintenance program? 27.If your Company is awarded the Regional Incentive Agreement, would you provide the pumps at no cost to the government? If yes, what is the ratio of sleeves usage that would warrant a free pump? Who would maintain the pump? 28.Do your sleeves come in a package that can be delivered to a sterile field? 29.What types of educational tools or materials do you have for this product line? 30.What kind of staff training does your company provide? 31.Does your company provide customer service 24/7? 32. Is there any history of backorders and/or recalls for this product group?
