- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is FM442752720676; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-10 and Defense DCN 20060616. This acquisition will be full and open competition; the North American Industry Classification System (NAICS) code is 339112. The business size standard is 500 employees. The Federal Supply Class (FSC) is 6515. The Standard Industrial Classification (SIC) is 3841. This request for quotation consists of the following Infusion Pump System: Item 0001: CADD-Prizm PCS II Pump model 6101 OR EQUAL: 25 each; For continuous, clinician activated bolus, and patient activated bolus infusions. Used for continuous infusion, programmed automatic dosing and demand dosing. Includes keypad, pump pouch, 9-volt alkaline battery, pump key, remote dose cord, carrying case, operator?s manual. Item 0002: Lockable Polemount Bracket OR EQUAL: 25 each. Item 0003: Tubing 60" with male luer, clamp, and integral anti-siphon valve with male luer: 25 EA; 1 Box/50 EA per box. Item 0004: Shipping cost to Travis AFB, CA, 94535: 1 EA. All equal items must meet or exceed the features of the items described. The delivery location is Travis AFB, California. Ensure you include the delivery time for project completion. Upon award the contractor will be required to coordinate delivery with the customer.
