Description
This is a Combined/Solicitation for Commercial Items in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6 as supplemented with additional information included with this notice. This notice constitutes the ONLY SOLICITATION. Price quotes are being requested and a written solicitation WILL NOT be issued. This solicitation HSCG81-06-Q-3BL093 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated Provisions and Clauses are those in effect through Federal Acquisition Circular 2005-08. FAR Clauses and Provisions are available via internet address: http://www.arnet.gov/far/. This RFQ is being solicited as SET-ASIDE Small, Woman-Owned, Service-Disabled Veteran-Owned, HubZone, Small Disadvantaged Veteran-Owned, business etc. The North American Industry Classification System is 334516 and Small Business Size Standard is 500 Employees. Item Description and Quantities are as indicated: TSI Universal Portacount Systems, to include the following: A.) Model 8020 Portacount Plus Respirator Fit Tester w/Carrying Case, AC Adapter, Alcohol Supply, FitPlus Fit Test Software on CD, Operation & Service Manual, Training Video, RS-232 Computer Interface Cable, B.) N95 Companion including Model 8026 Particle Generator, C.) Alcohol Cartridge, D.) Alcohol Fill Capsule, E.) Storage Cap, F.) Two (2) ea. HEPA Filters, G.) 3/16 in. Hose Adapter, H.) 1/4 in. Hose Adapter, and I.) Two (2) ea. Spare Alcohol Wicks. Quantities are as indicated: 7 complete Systems. Acceptance: Destination, F.O.B: Destination, Delivery expected at least 10 days from the date contract is issued. Delivery shall be to the following: Commander (kse), Norfolk, VA 23510, Commanding Officer (kse), Boston, Ma 02109, Commanding Officer (kse), St. Louis, MO 63103, Commanding Officer (kse), Portsmouth, VA 23703, Commanding Officer (kse), Opa-Locka, FL 33054, Commanding Officer (kse), New Orleans, LA 70146, and Commanding Officer (kse), Cleveland, OH, 44199. Federal Acquisition Regulation (FAR) Provisions that applies to this RFQ: 52.212-1, Instruction to Offerors-Commercial Items (Jan 2006), 52.212-2, Evaluation-Commercial Items (Jan 1999), and 52.215-5, Facsimile Proposals (Oct 1997). The Government will award a contract resulting from this solicitation to the responsive, responsible offeror whose offer conforming to this solicitation will be most advantageous to the Government, Price and other factors considered. The following factors shall be used to evaluate offers: Price, Delivery, and Past Performance Factors (please provide Points of Contact, Telephone Numbers, and the types of Supplies of which contracts have been satisfactorily completed of at least 3 current references). 52.212-3, Offerors Representations and Certifications-Commercial Items (Mar 2005) (NOTE: As of Jan. 1, 2005 ORCA has become mandatory and is available through the Business Partner Network (BPN) at http://www.bpn.gov/. A hard copy is no longer required if Contractor is ORCA registered, if NOT registered, the contractor can upload their Reps & Certs to ORCA. Contractor must have an active Central Contractor Registration (CCR) record and a Marketing Partner Identification Number (MPIN). To search for the contractor's Reps & Certs, you must go to this page http://orca.bpn.gov/publicsearch.aspx and enter the contractor's DUNS number.) NO CONSIDERATION WILL BE GIVEN TO QUOTES LACKING THIS AND ANY OTHER INFORMATION SET FORTH IN THIS SOLICITATION. Fedral Acquisition Regulations (FAR) Clauses that applies to this RFQ: 52.212-4, Contract Terms and Conditions-Commercial Items (Sept 2005), 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items (Feb 2006) applies as follows: paragraph (b)(1), (7), (14), (15), (16), (17), (18), (19), (20), (23), and (31). In compliance with the said Clauses, the following additional FAR Clauses 52.252-2, Clauses Incorporated by Reference (Feb 1998) as part of and apply to the RFQ: 52.247-34, F.O.B. Destination, (Nov 1991). PLEASE READ, COMPLETE, AND PROVIDE ALL INFORMATION REQUESTED IN THIS SOLICITATION PARTICULARLY FAR PROVISION(S) 52.212-1 and 52.212-3.
