Description
This Federal Correctional Institution, Waseca, Minnesota will be issuing a solicitation to obtain Optometric Services. The provider shall deliver services to include, but not limited to, the examination, diagnosis, and treatment of eye disorders, recommendations to the Clinical Director for appropriate optometric care, treatment for ocular disease, prescribe eyewear and any medically indicated features of eyewear for patients?s unique ocular needs, perform tonometries and slit lamp examinations when medically indicated/requested, and make necessary adjustments of institution eyeglasses and frames. Shall make recommendations to the Health Services Administrator and Clinical Director for the treatment of eye diseases and the procurement of eyewear. The provider will follow standard infection control practices, including the mandates of the concept of Universal Precautions and applicable OSHA guidelines. Performance Schedule: The performance shall be one (1), four (4) hour session, four (4) times per month, with the option for additional sessions each month, not to exceed a total of (55) sessions for this contract period, inclusive of one session dedicated to contractor orientation. The government reserves the right to use the lesser amount, or exercise the option, as dictated by the needs of the department. The provider will be required to see a minimum of twelve (12) patients per session as determined by the Health Services Administrator or his designee. Session shall be scheduled Monday through Friday, exclusive of Federal holidays, during the normal administrative hours of 7:30 A.M. through 4:00 P.M. The sessions shall be mutually agreed upon by the Health Services Administrator and the provider. Multiple sessions can be performed on the same day within the prescribed time constraints, and dependent upon the needs of the institution. The period of performance shall be from the date of award through September 30, 2007. Award is anticipated to be October 1, 2006 based on the availability of funds. The solicitation will be available on or after July 18, 2006 and will be distributed solely through the General Services Administration's Federal Business Opportunities web site http://www.fedbizopps.gov/. Hard copies of the solicitation document will not be available. The site provides downloading instructions. All future information about this acquisition, including solicitation amendments, will also be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. Faith-based and community-based organizations have the right to submit offers/bids equally with other organizations for contracts for which they are eligible.
