Description

Furnish all labor, transportation, materials, tools, equipment and supervision to provide home medical equipment services to beneficiaries of the Veterans Affairs Medical Center, Atlanta, Georgia. Services shall include the assembly, delivery, pickup, cleaning, repairing and storage of Government-owned home medical medical equipment. Contract period to be October 1, 2006 through September 30, 2007, with four one-year options to extens. This procurement is being conducted under FAR Part 12 and FAR Part 15. The solicitation will be issued as a Request for Proposal (RFP). A technical proposal will be required. Technical evaluation criteria condists of: 1. Past Performance and Business Plan; 2. Quality Assurance; 3. Personnel Qualifications; 4. Small Disadvantaged Business (SDB) Participation; 5. Cost/Pricing. The NAICS Code is 621610 and the size standard is $12.5 million. The anticipated issue date is July 14, 2006. Request for hard copy solicitation and/or inquiries must be in writing to the Contracting Officer and will be accepted by e-mail at Faye.Thomas@med.va.gov or by fax at 478-274-5581. Any amendments issued to the solicitation will be available at the URL address. Interested parties are advised that they are responsible for obtaining amendments.
