- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Written quotations are being requested. Quotes may be submitted on the Standard Form 18 (Request for Quotation). STATEMENT OF WORK FOR MEDICAL SERVICES GENERAL The contractor shall furnish a Board Certified Occupational Medicine Physician to provide services for approximately fifty (50) employees for the U.S. Environmental Protection Agency, Gulf Ecology Division (GED) Employees, at 1 Sabine Island Drive, Gulf Breeze FL. The Gulf Ecology Division is responsible for research on the physical, chemical and biological dynamics of coastal wetlands and estuaries, to determine ecological condition, evaluate rates and causes of declining systems, and predict future conditions under various alternative water quality scenarios. The purpose of these medical examinations is to ensure that EPA Employees working in laboratories have not suffered adverse health effects. A list of examination requirements will be provided upon request. GOVERNMENT FURNISHED ITEMS The government shall furnish the EPA Medical Evaluation Form. A copy of the form will be provided upon request. Contractor Furnished Items and Service Specific Tasks The contractor shall provide medical exams and services for employees when requested by the GED Safety, and Occupational Health Manager. Employees are regularly scheduled for baseline and other examinations. An employee may require evaluation at times other than the scheduled examination time. The required procedures for each examination are listed on the attached forms. The contractor shall also perform international travel physical and immunizations. The contractor shall provide information as to the location where each aspect of the physical exams will be performed, price to cover each aspect of the exams and must indicate how many appointments would be necessary to perform all aspects of the exams. The contractor shall provide the employee with the option of seeing the same physician for annual and biennial exams. The contractor shall provide the Safety and Occupational Health Manager a monthly report consisting of employees examined during the month by names and dates. The contractor shall complete all medical forms provided by the Safety And Occupational Health Manager. A copy of this form is attached. The laboratory will provide any updates to the forms. The contractor shall retain the original copy of the completed forms as a part of the employee?s medical records. These forms are strictly confidential. The employee?s medical records are covered by the Privacy Act of 1974 and other regulations that address and protect the confidentiality of this information. Medical records shall remain with the provider until written notice is received to transfer or forward the records to a different location. Individuals may obtain a copy of the information contained in his/her medical records upon request, and may request that copies be sent to their personal physician. Upon initiation of this agreement, the contractor shall have the responsibility of receiving all previous GED records. The contractor shall serve as the Medical Review Officer (MRO) and fill out the 10th page of the EPA Evaluation Form for all participants except for the divers. The contractor shall be responsible for maintaining the original EPA Evaluation Form and forwarding a copy of the completed form for all divers to the EPA Federal Occupational Health doctor. The contractor shall forward one copy of page 10 for all other patients to the Safety And Occupational Health Manager. When requested by the Safety And Occupational Health Manager, the contractor shall discuss health hazards and toxic effects of substances used at the laboratory and shall research specific areas of preventative medicine and toxicology. The contractor shall also provide the Safety and Occupational Health Manager with suggested solutions for areas needing improvement. This time spent on research shall be considered consultation hours. TERMS OF THE AGREEMENT The total amount of all calls placed against this Agreement shall not exceed $100,000 over a period of five years. The vendor shall notify the Contracting Officer in writing whenever they have reason to believe that the next call order placed against the Blanket Purchase Agreement (BPA) will exceed 75% of $100,000. The government is not obligated to reimburse the vendor for any costs that exceed $100,000. The vendor is not obligated to accept calls placed against this BPA in excess of $100,000. The vendor warrants by performance that any call placed under this Agreement shall be at the price or at a lower price than those charged to their most favored customer for same or comparable service under similar terms and conditions, including discounts for prompt payments. The issuance of individual calls on the open market under this agreement will be made in accordance with the Federal Acquisition Regulations. Calls placed under this Agreement will made oral or in writing by the ordering officer. Each call will be assigned an identification number. The number will identify the fiscal year the call is placed and the consecutive call number of the action (call) within the fiscal year. Example: The first call place in fiscal year 2007 shall be designated 07-01. The first two digits (01) designate the year. Should 34 calls be placed between October 1 and September 30, the final call shall be 07-34. On October 1, 2008 (the initial day of fiscal year 2004) the numbering system shall begin with 08-01. All Calls placed with the vendor under this agreement must be accompanied by a BPA call number authorized by the Contracting Officer. Upon award, a list of individuals will be provided to the contractor with the names of all authorized individuals, which can place calls. The contractor shall ONLY accept calls from the individuals provided on the list. Any calls placed by individuals other than those authorized callers listed above will be considered as unauthorized procurements and the unauthorized caller may be liable for the cost of the services or supplies provided. A requirement that all services under the agreement shall be accompanied by a receipt that shall contain the following formation: I. Type of service. II. Patient Name III. BPA number. IV. Date of exam. V. Itemized list of services. VI. Date of treatment VII. Blanket purchase agreement call number VIII. Name of EPA individual placing the order The vendor shall submit a summary invoice for the period ending the 24th of each month. Any transaction after the 24th of each month shall be submitted on the next month summary invoice. A summary invoice shall be submitted at least monthly or upon expiration of this BPA, whichever occurs first, for all services made during a billing period, identifying the delivery tickets covered therein, stating their total dollar value, and supported by receipt copies. ONLY A SUMMARY INVOICE WILL BE ACCEPTED FOR EACH MONTH UNDER THE TERMS OF THIS AGREEMENT. ALL INVOICES RECEIVED OTHER THAN SUMMARY INVOICES WILL BE RETURNED TO THE VENDOR. 52.252-2 -- Clauses Incorporated by Reference (Feb 1998). This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available.
