Description
Medical Supplies - Potential Sources Sought Military Hospitals: (Army, Navy, Air Force) Southwest Region SURGICAL INSTRUMENT CLEANERS & LUBRICANTS: Standardization Point of Contact: Cindy Ingrao, Clinical Consultant, Tri-Service Regional Business Office, Southwest Region, San Diego, CA. Telephone: (619) 532-8517. Potential sources sought to participate in the regional standardization of Surgical Instrument Cleaners & Lubricants. Southwest Region, a military integrated delivery network (IDN), comprised of Army, Navy and Air Force medical treatment centers in California are in the process of standardizing Surgical Instrument Cleaners & Lubricants. If your company currently holds or is in the process of obtaining a Distribution and Pricing Agreement (DAPA) with the Defense Supply Center-Philadelphia (DSCP) on Surgical Instrument Cleaners & Lubricants, your company is eligible to participate in the standardization process. Please respond by Email: cingrao@nmcsd.med.navy.mil and provide your company name, address, point of contact, telephone, fax and E-mail address and answers to the below listed questions no later than August 9, 2006. For questions, please call (619) 532-8517. 1.Does your company manufacture and/or distribute a complete line of surgical instrument detergents/cleaners and lubricants? 2.Are your products available through the Prime Vendor, Cardinal? 3.What is your DAPA Number? 4.Under what brand(s) does your company manufacture or distribute your products? 5.Under what standards are your products manufactured? 6.Please provide a list of the products your company offers. 7.How are your products packaged? 8.Do your detergents/cleaners/lubricants come in solid to liquid or liquid form? 9.Do you have detergents/cleaners for use with different types of water supplies? If so, list the different types of detergents/cleaners and the corresponding types of water supplies. 10.Can your detergents/cleaners/lubricants be used in all washers/disinfectors currently on the market? If not, which washers/disinfectors your products can?t be used in? 11.Do your detergents/cleaners/lubricants remove iron oxide and mineral deposits, proteins, lipids, blood, and carbohydrates? 12.Do your products possess organic sequestering agents to manage water hardness and mineral deposits? 13.Do your products possess corrosion inhibitors to minimize corrosion and discoloration of instruments? 14.Are your detergents/cleaners bacteriostatic and odor suppressing? 15.What is the efficacy of your detergents/cleaners? 16.Does the efficacy of solutions have to be determined after several uses or is there a maximum time the solution is effective? 17.Is there documentation of an acceptable level of efficacy for the removal of particulate matter/contaminating organisms? Please provide that documentation in an electronic format. 18.Do any of your products require mixing or activation? 19.What is the shelf life of your products prior to opening or activation? 20.What is the use life of your products once opened or activated? If the life is different between the two, what is the use life of your products once placed into the washing receptacle? 21.Do your products require storage at certain temperatures? 22.Are your products active in temperatures up to 120 degrees Fahrenheit? 23.Are your detergents/cleaners non-corrosive and non-staining? 24.Are your detergents/cleaners safe for all instruments and equipment including scopes made of all materials? If not, which ones? 25.Can any of your products damage instruments left to soak past your recommended time? 26.Does your company provide timers with your products? 27.Do your products require any treatment prior to disposal in a sanitary sewer in California? If so, what is the process for treatment and disposal? 28.Does your company provide this training at no charge to the facility? 29.Please provide the MSDS for all of your products in an electronic format. 30.Are Permissible Exposure Limits established for any of your products? 31.Under NIOSH, ANSI/AAMI, California or Federal OSHA or any other regulatory or advisory agency, please state which of your products are recommended or required to have: a.Personal protective equipment. What type? b.Eye wash station c.Safety shower d.Specified ventilation e.Special storage (e.g., flammables) f.Exposure monitoring of personnel 32.What types of educational tools or materials do you have for this product line? 33.What kind of staff training does your company provide? 34.Does your company provide customer service 24/7? 35.If your company is awarded the Regional Incentive Agreement, will wall mounts, floor stand holders, and vacuum hose be provided at no charge for conversion? 36. Is there any history of backorders and/or recalls for this product group?
