Description
The Government intends to award a Firm Fixed price contract for non-personal commercial services for the development of forms NAVMED 6260/7 (Rev. 5-90) and NAVMED 6260/5 (Rev. 5-90) for web-based reporting of B-readings and history/physical exams and the implementation of their use to include designing, set-up and testing of server using the reporting forms. The selected contractor must provide professional, technical, administrative and program management support to The Occupational and Environmental Medicine Directorate (OEM) located at the Navy Environmental Health Center (NEHC), Portsmouth, VA. The period of performance is for one (1) year from date of contract award. The federal supply code is 7030. The applicable North American Industry Classification System (NAICS) Code is 541511 and the size standard is $6.5.million. Proposals will be evaluated for past performance and price. A complete RFQ package will be available on or about 14 July 2006 by downloading! the documents at http://www.neco.navy.mil or http://www.fedbizopps.gov. Potential offerors are hereby notified that the solicitation and any subsequent amendments will only be available from download at the above sites; this office will no longer issue hard copy solicitations. By submitting an offer, the contractor is self-certifying that neither the requestor nor the principal corporate officials and owners are currently suspended, debarred or otherwise ineligible to receive contracts from any federal agency. Failure to do so may represent grounds for refusing to accept the offer. By submission of an offer, the contractor acknowledges the requirement that a prospective awardee must be registered in the CCR database prior to award, during performance, and through final payment of any contract resulting form this solicitation, except awards to foreign vendors for work to be performed outside the United States. Lack of registration in the CCR database will make a contractor ineligible for award.
