Description
This modification #2 is to post the questions and answers received. This modification does not change the date for responses due. Question (1) The walk through focused on the labs, will there be an inventory of all the items going to surplus? Answer (1)No inventory exists. The SOW provides the number of personnel moving and the office areas can be viewed during the second site visit on Wednesday July 26th. Question (2)Will we be able to go back to the labs at some point prior to award with our riggers to review the technical equipment? Answer (2) Yes, see attached schedule for second site visit on Wednesday July 26th. Question (3) Who will disconnect the hoods from the wall and or ceilings? Answer (3) The move Contractor is responsible for arranging all disconnects and reconnects. Question (4) Are we responsible for moving the hanging wire management systems we viewed is several locations? Answer (4) No, The hanging wire management is not moving. Question (5) Are we moving any HAZMAT materials? Answer (5) Yes, example pumps and compressors that have contained oil. The Move Contractor is not responsible for moving chemicals. Question (6) Does some of the lasers have to be moved in a ?vacuumed? environment? Answer (6) There are pieces of equipment that are internally under vacuum and need to stay under vacuum at all times. The environment in which the lasers are transported does not have to be under vacuum but all optical equipment shall be moved in a clean environment. Question (7) For insurance purposes and the cost associated with purchasing additional insurance can we get a dollar value and a list of the items valued over 5k? Answer (7) Cost information will be provided at the July 26th 2006 Site visit. Question (8) On Phase III there is a move for 150 personnel and equipment. Do you firm have an inventory of all that is moving? Answer (8) Phase III is the relocate 150 staff and their contents and equipment as well as the laboratory contents. There is on office contents inventory. Question (9) Is there a date that the contractors can bring in a rigging company to look at the thing we need to relocate? Answer (9) Yes, see attached schedule for site visit on Wednesday July 26, 2006. Question (10) In the SOL page 5 of the Statement of work FDA Library on area said that we will relocate 1481lf, will their be a change order to that so it will be noted for all vendors to quote on Answer (10) Revised estimated linier feet for libraries: Parklawn 12,000, 65%-70% will move to White Oak remainder will be relocated within Parklawn library, Wilkins no change. 9200 Corporate Blvd. 4,500, 3,300 move with 60% to White Oak and 40% to Parklawn. ADDITIONAL CLARIFICATIONS: Library move schedule: 8/28/2006 Deliver crates to library staff; 9/9/2006-9/13/2006 Relocate library Collection and equipment from Building 22 to CSU; 9/11/2006 Move Library staff from Parklawn (8); 9200 Corporate (1) and Building 22(3) to CSU library. 9/23/2006-9/29/2006 Move library collection & equipment from Parklawn to White Oak CSU Library; 9/28/2006 Relocate library staff (2) from 9200 Corporate to Parklawn; 10/2/2006-10/5/2006 Move library contents From 9200 Corporate to White Oak CSU Library; 10/4/2006-10/6/2006 Relocate library contents internally within Parklawn library; 10/10/2006-10/12/2006 Relocate library contents from 9200 Corporate to Parklawn; 10/12/2006-10/15/2006 Relocate library collection from Wilkins Av. to Parklawn. These statements of work may provide guidance on some specific equipment that needs to be handled by specialty vendors. Statement of Work - Disassembly, Relocation, and Installation of Dark Room Systems From 12725 Twinbrook, Rm 170 To White Oak Campus. Disassemble, relocate reinstall, and test dark room system comprised of the following items: Ilford 2150 and base; Fuji Pictography 3500; Kreonite Sink 75-1/2 x 32-1/2; Kreonite EM sink; Durts Laboratory Enlarger S-45EM; 75? shelf; 6- 24? base and wall cabinets; 1- 36? base and wall cabinet; Counter top; STATEMENT OF WORK - Onsite Calibration of ELF 3300 Mechanical Test System at White Oak Campus. The following tasks must be completed by a qualified vendor for these items: Calibrate of systems; Equipment; Bose ElectroForce ELF Equipment (2 units); Statement of Work Disassembly and Reassembly of EMCO GTEM System; From 12725 Twinbrook, Rm 165 To White Oak Campus. The following tasks must be completed by a qualified vendor for the items: Disassemble and crate EMCO GTEM System; Reassemble EMCO GTEM System at new location; Perform VSWR and TDR Measurements. Statement of Work - Disassembly and Reassembly of Instron 4400R/1125 and Instron 4465 From 12725 Twinbrook, Rm 118 To White Oak Campus. The following are the tasks required to relocate the Instorn 4400R/1125 and Instron 4465. Qualified vendors must be used to maintain warrantees on the units. Disconnect and package units; Reinstall and Verify. Verifications: 4400R/1125, 2,000 gram, Compression Only 50 Hg, Compression Only; 50 Kg Tension Only; 500 Kg Reversible; 10,000 KG Reversible; 30 kn Reversible; 4465 1 KN Reversible; 5KN Reversible; 5 KN Reversible; 100 N Reversible; 100 N Reversible; Video Extensometer 2 Lens Static Only; Calibration Statement of Work Disassembly, Relocation and Reassembly of X-Ray System #1 From Twinbrook Bldg #1, Rms 170, 175, 182 To White Oak. Overview: Five X-Ray systems in FDA?s Twinbrook Parkway Building #1 Facility must be relocated to the Engineering Physics Building on the White Oak Campus. These systems are sophisticated and composed of many subassemblies. Whoever relocates this equipment must be knowledgeable and experienced in X-Ray generation and control. Description of Existing Equipment: The X-Ray systems and a list of their subassemblies as they currently exist are listed below: (1)Fisher Mammography System in Room 182 to be relocated to White Oak 3129; (a)Generator and C-Arm; (b) X-Ray Tube (removed from C-Arm); (c)Grid (removed from C-Arm); (d) High-speed rotor controller; (e) Generator; (f)Interconnecting Cables. (2)GE Single Phase System in Room 182 to be relocated to White Oak 3130 (a)Control Console; (b)X-Ray Tube; (c) X-Ray Tube mount/stand; (d)High-voltage tank; (e)Voltage dividers (two small tanks); (f)Interconnecting Cables; (3)Transworld System in Room 175 to be relocated to White Oak 3129; (a)Control Console; (b)X-Ray Tubes (three); (c)X-Ray Tube mount/stand; (d) High-voltage tanks (three); (e)Voltage divider; (f)Interconnecting Cables. (4)CPI Indico 80 kW System in Room 170 to be relocated to White Oak 3124; (a)Control Console; (b)X-Ray Tube; (c)X-Ray Tube; Mount/stand; (d)Cooling Apparatus; (e)Generator; (f) Interconnecting Cables; (5)Continental System in Room 170 to be relocated to White Oak 3130: (a)Control Console; (b)X-Ray Tube; (c) X-Ray Tube mount/stand; (d)Generator; (e)Interconnecting Cables. Statements of Work for All Systems Listed Above: Disassemble the X-Ray System; Move the existing X-Ray System components to White Oak; Reassemble the X-Ray system (including power and interconnections; Realign and calibrate the X-Ray system; Certify the relocated X-Ray system for operation within its specifications. Please note that all associated optical benches/beams will be moved by a rigging firm separate from the work described here. All material/equipment are to be delivered directly to the rooms at White Oak were these goods will be installed/used. The vendor will be responsible for getting everything to the installation sites without help from FDA personnel. The vendor will be responsible for getting dock delivered goods from the dock to the rooms where the goods will be used. A notice to proceed will be given at least 30 days prior to site access availability. All sites are expected to be available sometime between December 1, 2006 and February 1, 2007. Electronic drawings of the receiving sites will be supplied and the vendor will be expected to review these drawing and commit to fulfilling the contract work requirements. An original and 3 copy of the offerors cost and technical proposal shall be received on or before 1700 EST local time, 28 July, 2006 at Food and Drug Administration 5630 Fishers Lane Rockville, MD 20857 via e-mail only.
