Description
COMBINED SYNOPSIS/SOLICITATION Install Additional Locks and Reprogram Cameras (i) This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation is issued as a request for quote (RFQ). Submit written quotes (oral quotes will not be accepted) on RFQ reference number F2WA16171A100-HEIM. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-11. (iv) This procurement is being issued as a 100% small business set-aside. The North American Industry Classification System (NAICS) code is 238210 with a small business size standard of $13M. (v) This combined synopsis and solicitation is to purchase the installation of additional locks and reprogram cameras for the 341st Medical Group. 1) General Requirements: a) This project consists of two modules. b) Contractor is responsible for all materials, labor, permits, management oversight, and equipment to accomplish the renovations. c) The contractor is responsible for the removal of all debris, trash and construction residue to an off base location for proper disposal. d) Contractor must abide by current NFPA, OSHA, International Building Code, Uniform Building Code, DOD, US Air Force and Malmstrom AFB requirements. e) Contractors shall be required to attend a Safety and an Infection Control Briefings prior to start of work. f) Contractor must consider the impact their work will have on Patient Care and keep that impact to absolute minimum. g) Work will be completed daily between the hours of 0700-1630, except for weekends and Federal Holidays as approved in advance by Facility Manager or Administrator. h) Contractor?s personnel shall; participate in fire drills and all other actual emergencies at the direction of the Facility Manager or the on-scene-commander, wear clinic identification badges at all time while within clinic i) Dust and infection control measures must be taken to prevent the spread of the dust and contaminants out of construction area. Contractor must submit a written risk management plan for infection and dust control before work begins. j) Medical Group Facilities are ?Zero Tobacco?. Smoking is allowed only in the designated smoking area behind the ambulance garage or private vehicle. Smoking is not allowed within 25 feet of any door. k) Proposals will be submitted in modules. l) All HVAC controls will be compatible with and connected to current HVAC main control system m) Door frames must be grouted in place. n) New doors must match existing red oak doors with the following specifications i) Graham Premium wood doors ii) Prefinished #500 Medium Red iii) Prep for 3ea 4 1/2'? Hinges Square Corner iv) Prep for mortise Lock (if required), Sectional Trim v) All Doors poly bagged vi) All Doors mark with door I.D Number vii) Doors with Vision Lites to have metal vision (painted to match existing) w/ safety glass installed o) Door hardware will match exiting Schlage units p) Any door with DSX Access control currently installed will be reconnected to existing system and operation verified. q) Contractor will provide at no additional cost all appropriate manuals, software, databases, and cables for all devices/items installed. r) Any electrical/mechanical/structural/utility change will be annotated on as-built drawings and labeled accordingly. 2) Module 1; Install locks to 15 doors to existing DSX Access control system a) Update database, graphics and verify operations after upgrade. b) Current software version WinDSX 3.7.10 3) Module 2; Reprogram Pan, Tilt, and Zoom (PTZ) cameras a) Programming must enable operator to manipulate existing PTZ cameras from current software WaveReader Ver. 4.0.23 (vi) Delivery requested no later than 60 days after receipt of order. (viii) Quotes will be evaluated on price, price related factors and ability to meet specifications. (ix) The offeror must include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications ? Commercial Items. (x) The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this solicitation.
