THIS IS A SOURCES SOUGHT NOTICE. FOR FURTHER INFORMATION CONTACT MS GINNY SLAYTON, TEAM LEADER LOGISTICS AT 706-787-2019. This FEDERAL BUSINESS OPPORTUNITY announcement is a notice that TRICARE Southeast, which includes all DoD (Army, Air Force, Nav y) Medical Treatment Facilities (MTFs) in Alabama, Georgia, Florida, Mississippi, Tennessee and South Carolina are in the process of standardizing TROCARS as listed in the subject line above. Companies who currently possess or are in the process of obtain ing a Distribution and Pricing Agreements (DAPAs) with the Defense Supply Center, Philadelphia (DSCP) for this product are eligible to participate in the standardization process. For additional information on obtaining a DAPA, you may call DSCP at (215) 7 37-8740. Base company criteria for this product Line: TROCARS includes but is not limited to the following: must carry a FULL LINE OF TROCARS that can be used on all patient populations (i.e., pediatric, adult, bariatric), must provide customer support in the form of a sales representative for each MTF within the TRICARE Southeast Region, must provide clinical support, education programs, and utilization reports, must provide companys return policy, must provide companys distribution network, must be a ble to provide product for all MTFs within the TRICARE Southeast Region, must provide volume discounts, and must provide electronic and hard-copy of proposed pricing and competitive product cross-reference. Must be able to provide representatives and prod uct for vendor demonstrations at no charge to the government and conduct product in-services for medical treatment facilities as needed. Must have or be in the process of obtaining a DAPA Number before the evaluation phase begins. Base technical criteria for this product line: Provide your Company Name, Point of contact and address. Does your company manufacture or distribute a full product line in a variety of sizes of TROCARS? (i.e., pediatric, adult, bariatric) List the type of products and sizes. Provide any new technology that your company currently has. Does your company have a DAPA number? IF so what is it? If your company does not have a DAPA number, provide date applied for. Are your products on the Federal Supply Schedule? Are your products available through the Prime Vendor, Owens & Minor? If not, provide date applied for. Under what brand(s) does your company manufacture or distribute your products? Are your Products FDA approved? Under what standards are your products manufactured? What testing was used to support the efficacy of your products? Does your company manufacture/distribute a full line of disposable self reducing trocars, including long trocars? If so, list the types and sizes, item nomenclature, Universal Product Number (UPN), Item Product Number/Catalog Number, and cross reference av ailable to competitive products. What type of materials are your trocars made from? Is your product latex free? Do your seals allow adaptability to all size scopes/instruments? What if any disposal issues are there? Do you use non toxic, color fast dyes for labeling your products? Packaging features: Expiration date visible? Lot number? Shelf life in years? Color coded by size? Length of time product has been on the market? Have there been safety alerts for your trocars or reducers? Does your company have the ability to service the entire Region? What is the return goods policy? Any history of backorders and/or recalls for this product group? If yes, please list dates and duration length, cause, and resolution. Do you offer clinical support? Does your company provide customer service 24/7? Do you have customer service representatives for each Medical Treatment Facility? What types of educational tools/materials do you have for the product line? Provide your (1) company name, (2) address, (3) point of contact, (4) telephone number, (5) fax number, (6) e-mail addr ess, and (7) your DAPA number for the product(s)/product line(s) being offered, (8) and answers to Ginny Slayton; virginia.holeman@amedd.army.mil. Respondents must provide all written detailed information on the Trocars, which clearly outlines and supports /verifies the requirements stated above no later than 21 days after the posting of this announcement.
