Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This solicitation uses Simplified Acquisition Procedures (SAP) authorized in FAR Subpart 13.5 and is expected to result in the award of one firm-fixed price contract. The resulting award will be made on SF 1449, for commercial items. This announcement constitutes the only solicitation; quotations are being requeste d and a written solicitation will not be issued. W91YTZ-06-T-0128 is a request for quotation (RFQ); its document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2001-23 and 2001-19 Update. This is an unrestricted procurement. The Standard Industrial Classification Code is 3845 and the small business size standard is 500. The North American Industry Classification System Code is 334510. Contractor shall provide Anesthetic Gas Modules for the Department of Surgery, Womack Army Medical Center. The m anufacturer of the monitoring systems is Philips Medical Systems, Andover, MA 01810-1099. The Anesthetic Gas Modules are used in conjunction with anesthesia machines to monitor and display several parameters to include vital signs, hemodynamic parameters , end tital C02, and inhalation anesthetics. The Government intends to make a sole source award for services described herein to Philips Medical Systems. All offers received in response to this solicitation will be considered. Responses will be used to determine whether bonafide competition exists. PERFORMANCE WORK STATEMENT: ITEM 0001: Anesthetic Gas Module M1026BT Order must meet all hospital requirements and be shipped in its entirety FOB, Fort Bragg, NC prior to payment. 10 ea UNIT PRICE___________TOTAL AMOUNT__________. ITEM 0003: Anesthetic Gas Module M1026BT. Order must meet all hospital requirements and be shipped in its entirety FOB, Fort Bragg, NC prior to payment. 4 ea UNIT PRICE___________TOTAL AMOUNT __________. ITEM 0004: K11 I-VUE interface CBL 1.0m-K11. Order must meet all hospital requirements and be shipped in its entirety FOB, Fort Bragg, NC prior to payment. 4 ea UNIT PRICE___________TOTAL AMOUNT___________. Womack Army Medical Center, Fort Bragg, North Carolina, 28310, intends to issue a fixed-price contract for anesthetic gas modules. Offers will be based on furnishing Philips Medical Systems upgrades. The anesthetic gas modules are for Womack Army Medic al Center (WAMC), Fort Bragg, NC. Gas modules will be installed at Womack Army Medical Center, Building 4-2817, Reilly Road, Fort Bragg, NC 29310-5000, FOB destination.
