- This is a sole source commercial service. This is also an unrestricted combined synopsis/solicitation for commercial service in accordance with FAR Part 12.6, as supplemented with additional information included in this notice. This announcement constitutes th only notice of this Request for Quote (RFQ) and a written solicitation will not be issued. This procurement is SUBJECT TO AVAILABILITY (SAF). This combined synopsis/solicitation reference number is W911SE-06-T-0040. The associated NAICS code is 334516 with a standard size of 500 employees. Offerors must proposes a fixed rate for 1 October 06 - 30 September 07 a 12 month period of performance. The contractor shall provide and furnish all labor, material, and parts required to perform full maintenance on the equipment set for in this contract. All services shall be performed on site in accordance with the manufacturer's specifications and best commercial practices with the least interference with the Government's operations. Validation shall be performed for capillary electrohoresis performed fo r the analysis instruments, reagents nad standards according to TWGDAM and DAB standards. This validation must then be documented and entered into the USACIL Lab Standard Operating Procedures and audited annually. The reagents and standards are purchased as DNA kits. The current validated instruments and are products of Applied Biosystems Incorporated which hold the patent rights for these products and a re the sole commercial distributor of the products. Full maintenance service shall include calls and all oncall maintenance required during the duration of the contract. Oncall maintenance includes all travel, labor, and repair parts required to correct malfunctions encountered in normal use of the equipment. Oncall maintenance and repair services required under this contract shall be requested by telephone. Government representatives authorized to place calls for oncall service are Lo rene Mosby or Tommy Rouse at (404) 362-7491. Upon receipt of a call for service, the contractor is required to repond within three workdays from the time of initial call. The contractor shall provide to USACIL, no less than 10 business days preceding the desired data of service, telephonic notice of his intension to service the covered equipment, notice shall be provided to Lorene Mosby or Tommy Rouse (404) 362-7491. The contractor shall furnish a service ticket for each service call and/or p reventive maintenance inspection. Contract items listed in the schedule under full maintenance. The service ticket shall be itemized to include serial numbers, be prepared in at least two copies, and signed by the COR. One copy shall be retained by the COR . All service ticket shall be signed by Lorene Mosby, Douglas Cooper, or Tommy Rouse to validated completion of the service.
