Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a more detailed solicitation is available on the Federal Technical Data Solutions (FEDTEDS) Web Page at http://www.fedteds.gov/ or at https://www.fedteds.gov/fedteds/start.nsf/frm.VendorLogin?OpenForm&SolicitationNumber=W912EE-06-T-00 45 , which includes exhibits that could not be posted as this site. Offerors should check the FEDTEDS Web Page often for new solicitations and/or changes (AMENDMENTS) to existing ones. Solicitation Number W912EE-06-T-0045 is being issued as a request for q uotation (RFQ) with the intent to award as a simplified acquisition. The solicitation document and incorporated provision and clauses are those in effect through Federal Acquisition Circular 2005-09. This procurement is SERVICE DISABLED VETERAN OWNED SMAL L BUSINESS SET-ASIDE (SDVOSB) under NAICS Code 212319 (SIC 1429) with a size standard of 500 employees. The USACE, Vicksburg District, Consolidated Contracting Office, proposes to award a simplified acquisition for the provision of all plant, labor, equipm ent, materials and supplies necessary to furnish and deliver approximately 25,450 tons of crushed stone to be delivered to six (6) locations. This procurement may be quoted as follows: Line Item 0001: Crushed Stone to be delivered to the following locations along the WEST BANK MISSISSIPPI RIVER LEVEE (MISSISSIPPI); Line Item 0001AA LOCATION A  Beaulah, MS (Bolivar County), Stations 1775 to 191 5, plus ramps at Stations 1786, 1828 and 1885: Quantity: 5,965 , Unit: Net Ton (2,000 LB), Unit Price $________, Total Amount Line Item 0001AA $________; Line Item 0001AB LOCATION B  Dennis Landing (Bolivar County, MS), Stations 428 to 526: Quantity: 3, 675 , Unit: Net Ton (2,000 LB), Unit Price $________ , Total Amount Line Item 0001AB $________; Total Amount for Line Item 0001 (Line Items 0001AA and 0001AB) $____________. Line Item 0002: Crushed Stone to be delivered to ARKANSAS RIVER LEVEE  NORTH BANK (ARKANSAS), Jefferson County between levee stations 2968+82 to 2835+62; Quantity: 3,330, Unit: Net Ton (2,000 LB), Unit Price $__________; Total Amount for Line Item 0002 $____________. Line Item 0003: Crushed Stone to be delivered to the following locations along the EAST BANK MISSISSIPPI RIVER LEVEE (LOUISIANA); Line Item 0003AA LOCATION A  East Carroll Parish (Louisiana), Stations 602+00 to 756+00: Quantity: 6,160 , Unit: Net Ton (2 ,000 LB), Unit Price $__________, Total Amount Line Item 0003AA $________; Line Item 0003AB LOCATION B  East Carroll Parish, Louisiana, Stations 878+00 to 947+00: Quantity: 2,760, Unit: Net Ton (2,000 LB), Unit Price $_______, Total Amount Line Item 00 03AB $________; Total Amount for Line Item 0003 (Line Items 0003AA and 0003AB) $____________. Line Item 0004 Supplies/Services: Crushed Stone to be delivered to ARKANSAS RIVER LEVEE  NORTH BANK (ARKANSAS), Jefferson County between levee stations 750+00 to 661+00; Quantity: 3,560, Unit: Net Ton (2,000 LB), Unit Price $__________; Total Amount fo r Line Item 0004 $____________. All stone shall be delivered by truck to the various delivery locations stated above. The contractor shall initiate delivery operations within seven (7) days after notification of contract award. Delivery shall be at the rate of not less than 500 tons pe r day, with all stone (approx. 25,450 tons) to be delivered NO LATER THAN 31 October 2006. Upon request to the contracting Officer, a site visit to the dumping sites may be arranged prior to the closing date of the solicitation. Measurement for all stone delivered to the site for payment will be accomplished by use of certified scales and provision of scale tickets to the governments on-site representative. The Contractor shall be responsible for obtaining any transport permits necessary and required for transport of material by way of county, state, or other public roads. Multiple awards may result from this solicitation. Award will be made for the items or combination of items that result in the lowest aggregate cost to the Government. Award shall be made to the responsive and responsible offeror whose quote is determined to be the lowest price. FAR Provision 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. Offerors must include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items with this offer. Offerors must also include a completed copy of the provision at DFARS 252.212-7000, Offeror Representations and Certifications-Commercial Items with this offer. The Clause 52.212-4, Contract Terms and Conditions, applies to this acquisition. The Clause at 52.212-5, Con tract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (Deviation), applies to this acquisition.
