Vendor shall provide delivery slips with all shipments and the delivery slip shall contain the following information: 1) Description of product received, 2) Exact quantities delivered, 3) Purchase Order Number, 4) and delivery slips shall be manually signed by an MTC staff other than person obtaining/issuing the call order. Prior to submitting an offer, you may visit the facility to make field measurements or evaluations.
