Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. NTC, Acquisition Command requirements request services to perform the following work required. (SEE STATEMENT OF WORK). This requirement is totally set-aside to small business. Only written (fax or email) proposals will be accepted. Proposals must be recei ved no later than 11:00 AM, 7 August 2006. POINT OF CONTACT: Annette Adams, 760-380-3788, fax 760-380-3693, annette.adams@irwin.army.mil. Statement of Work: Lodging, meals, use of facilities for 40 adults. Lodging based on adult double occupancy for participants (sliding scale for families with more than two individuals occupying a room), 7 meals per person, use of general conference room with coffee service a nd set-up included, use of all resort facitilites to include heated swimming pool, hot tub, exercise room, tennis courts, table games and grounds; inclusive of all tax and gratuities. For 4 Facilitators (double).Lodging based on adult double occupancy for facilitator couples, 7 meals per person, use of general conference room with coffee service and set-up included, use of allresort facilities to include heated swimming pool, hot tub, exercise room, tennis courts, table games and grounds; inclusive of all t ax and gratuities. For facilities for 2 Facilitators (single). Lodging based on adult single occupancy for facilitators, 7 meals per person, use of general conference room with coffee service and set-up included, use of all resort facilities to include he ated swimming pool, hot tub, exercise room, tennis courts, table games, and grounds; inclusive of all tax and gratuities. Group activities: challenge course activities to include professionally supervised high ropes activities as per safety regulations an d couple building activities for 5 hours for 40 adults, campfire with hot/cold beverages to include wood and all necessary safety supervision requirement for the fire, extra breakout room, audio visual rentals. Flat rate for up to 10 children. Period of pe rformance as follows: 24-26 August 2006, 7-9 September 2006, 28-30 September 2006. The Contractor shall submit one original copy of invoices to DFAS DFAS Lawton OPLOC ATTN: DFAS-LW-FPV 4700 Mow Way Rd, Dept 1791 Fort Sill, OK 73503 And one copy of invoice to Acquisition Command, Fort Irwin, CA 92310, ATTN: Claire Jones b. Payment will be made by DFAS, Fort Sill, Oklahoma (above address) c. Receiving Report , DD Form 250, will be prepared by the customer and submitted to DFAS Lawton, at above and e-mail a scan copy to Acquisition Command, ATTN: Claire Jones, clarita.jones@irwin.army.mil Contractor's invoice - A proper invoice must include the items listed below: (i) Name and address of the Contractor. (ii) Invoice date. (The Contractor is encouraged to date invoices as close as possible to the date of the mailing or transmission.) (iii) Contract number or other authorization for supplies delivered or services performed (including order number and contract line item number). (iv) Description, quantity, unit of measure, unit price, and extended price of supplies delivered or services performed. (v) Shipping and payment terms (e .g., shipment number and date of shipment, prompt payment discount terms). Bill of lading number and weight of shipment will be shown for shipments on Government bills of lading. (vi) Name and address of Contractor official to whom payment is to be sent (must be the same as that in the contract or in a proper notice of assignment). (vii) Name (where practicable), title, phone number, and mailing address of person to be notified in the event of a defective invoice. (viii) Any other information or documentation required by the contract (such as evidence of shipment).
