Description
This FEDERAL BUSINESS OPPORTUNITY announcement is a notice that TRICARE Heartland Region that include all DOD Medical Treatment Facilities (MTFs) in the states of Ohio, Kentucky, and Illinois are in the process of standardizing Safety Butterflies as listed in the subject line above. Companies must obtain, or be in the process of obtaining, a Distribution and Pricing Agreements (DAPAs) with the Defense Supply Center Philadelphia for these items to be eligible to participate in the standardization process. To learn more about the Regional Standardization program go to www.dscp.dla.mil/standardization . Please respond by Email: phyllis.targett@wpafb.af.mil and provide your company name, address, point of contact, telephone, fax and E-mail address and answers to the below listed questions no later than 21 days from the posting date. For questions, please call (937) 257-9075. Technical Criteria 1. Does your company manufacture and/or distribute a full line of safety butterflies? If so, please list the types of masks your company offers. 2. Are your products available through the Prime Vendor, Owens & Minor? 3. What is your DAPA Number? 4. Under what brand(s) does your company manufacture or distribute your product? 5. Are your products latex free? 6. Under what standards is your product manufactured? 7. Do your products meet safety standards as recommended by OSHA, NIOSH and AORN? 8. Can the safety feature be activated using a one-handed technique? 9. What types of educational tools or materials do you have for this product line? 10. Does the safety feature work well with a wide variety of hand sizes? 12. Does the safety feature interfere with normal use of the product? 13. What kind of staff training does your company provide? 14. Does your company provide customer service 24/7? 15. Is there any history of backorders and/or recalls for this product group? If yes, please answer: a. Dates and duration b. Cause c.
