- DESCRIPTION: THIS IS NOT A SOLICITATION ANNOUNCEMENT.     THIS IS A SOURCES SOUGHT SYNOPSIS ONLY.     THE PURPOSE OF THIS SOURCES SOUGHT SYNOPSIS IS TO GAIN KNOWLEDGE OF POTENTIAL QUALIFIED SOURCES AND THEIR SIZE CLASSIFICATIONS (HUBZONE 8(a), 8(a), HUBZONE, SMALL, SMALL DISADVANTAGED AND LARGE BUSINESS) RELATIVE TO NAICS CODE 541990(SIZE STANDARD OF $6.5 M).     RESPONSES TO THIS SOURCES SOUGHT SYNOPSIS WILL BE USED BY THE GOVERNMENT TO MAKE APPROPRIATE ACQUISTION DECISIONS-SEE SUBMITTAL REQUIREMENTS BELOW.     AFTER REVIEW OF THE RESPONSES TO THIS SOURCES SOUGHT ANNOUNCEMENT, A SOLICITATION ANNOUNCEMENT WILL BE PUBLISHED IN THE BOS.     RESPONSES TO THIS SOURCES SOUGHT ANNOUNCEMENT WILL NOT BE CONSIDERED AS AN ADEQUATE RESPONSE TO THE FORTHCOMING SOLICITATION ANNOUNCEMENT.     ALL INTERESTED OFFERORS MUST RESPOND TO THE SOLICITATION ANNOUNCEMENT IN ADDITION TO RESPONDING TO THIS SOURCES SOUGHT ANNOUNCEMENT.     The United States Department of Veterans Affairs, Veteran Integrated Service Network (VISN) 2, is seeking sources for a potential contract to provide necessary labor & materials to perform full maintenance & repair services on an Alcon Labs model 800CS Accurus Full Function Vitreoretinal Surgical System (XS4 Platform-II), s/n IACL001466, VA EE #117080. Contract includes 2 PM's per year (12 month period) and 2 375/Mackool hand piece exchanges. Responses to this notice should include company name, address, point of contact, Cage Code, size of business pursuant to North American Industrial Classification System (NAICS) standard 541990, with a size standard of $6.5 M, and must respond to the following questions: (1) is your business a large or small business in relation to the NAICS size standard? (2) If small, does your firm qualify as a small, emerging business, or a small, disadvantaged business? (3) If disadvantaged, specify under which disadvantaged group and is your firm certified under Section 8(a) of the Small Business Act? (4) Is your firm certified as "hub zone"? (5) Is your firm a woman-owned or woman-operated business? (6) Is your firm a certified Service-Disabled Veteran Owned and/or Veteran-Owned?     This is not a request for proposals and does not obligate the Government to issue a solicitation or award any contract.     Responses to this notice shall be mailed or faxed to the attention of Kellyann Bruno, Department of Veterans Affairs, 113 Holland Ave, Albany, NY 12208 If provided by fax, send to (518)626-6849 with a cover sheet identifying the attention name/address RFI (Request for Information) (Questions) shall be submitted by e-mail to Kellyann.bruno@va.gov . No phone calls. Responses must be received in writing no later than 4:30 local time, August 28,2006.     Firms responding to this announcement, failing to provide all of the required information cannot be used to help the Government make the acquisition decisions, which is the intent of this synopsis.     The purpose of this notice is to assist VISN 2 in determining potential sources only.     No solicitation is currently available.     All firms responding to this sources sought notice are advised that their response to this notice is not a request to be considered for a contract award.     If a solicitation is issued, it will be announced later in FedBizOpps and all interested parties must respond to that solicitation announcement separately from the responses to this announcement.     Responses to this sources sought synopsis will not result in inclusion to a prospective bidder's mailing list or to receive a copy of the solicitation.
