- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number F1B3R26215A100 is hereby issued as a Request for Quote. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-12 4 Aug 06. This acquisition is 100% set aside for small business concerns and is under North American Industry Classification Standards (NAICS) code 315211, with number of employees not to exceed 500. STATEMENT OF NEED: PROVIDE THE FOLLOWING PERSONAL DECONTAMINATION KITS: CLIN0001: Decontamination Kit, Personal (pre and post). Each packaged kit contains: ID wristband, white robe/labcoat, red plastic bag, scrub shirt and pant set, hard soled slippers, not less than 24? x 40? decon shower towel, clear plastic bag. Sizes: Medium ? 30 each; Large ? 75 each; Extra Large ? 75 each; Extra Extra Large ? 20 each. Total quantity: 200 each. Orex Item # PDK400 OR EQUAL. CLIN0002: Decontamination Kit, Personal (post). Each packaged kit contains: not less than 24? x 40? towel, modesty scrubs, hard soled slippers. Sizes: Medium ? 300 each; Large ? 750 each; Extra Large ? 750 each; Extra Extra Large ? 200 each.
