Description
This is a combined synopsis/solicitation for a commercial service prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The reference number F2F3316180A100; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-11 and Defense DCN 20060711. This is a small business set aside; the North American Industry Classification System (NAICS) code is 624190. The business size standard is $6,000,000. The Federal Supply Class (FSC) is R607. The Standard Industrial Classification (SIC) is 8322. This request for quotation consists of the following services: Item 0001: Qty 2080 Hours NTE, Period of Performance: 01 October 2006 through 30 September 2007 (Basic Year) Nonpersonal service; the contractor shall provide primary A&FRC customer intake, A&FRC program information and referral services and maintain the Volunteer Program (VRP) for Hurlburt Filed. Contractor shall provide all services as specified in this PWS. Item 0002: Qty 2080 Hours. Effective on 1 October 2007 through 30 September 2008. Period of Performance: 01 October 2007 through 30 September 2008 (Option Year One) The following provisions and/or clauses apply to this acquisition. FAR 52.222-41 Service Contract Act of 1965, Wage Determination number 94-3033 revision 05 dated 05/23/2006 is incorporated into this solicitation. FAR 52.222-42 Statement of Equivalent Rates for Federal Hires, In compliance with the Service Contract Act of 1965, as amended, and the regulations of the Secretary of Labor (29 CFR Part 4), this clause identifies the classes of service employees expected to be employed under the contract and states the wages and fringe benefits payable to each if they were employed by the contracting agency subject to the provisions of 5 U.S.C. 5341 or 5332. This Statement is for information only; it is not a wage determination, with the following information filled-in: General Clerk III, $10.03 per hour. This service is restricted to individuals who reside within a 50-mile radius of Fort Walton Beach, Fl. FAR 52.222-44 Fair Labor Standards Act and Service Contract Act, SECURITY REQUIREMENTS: All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the governmental installation, shall abide by all security regulations of the installation. Responsible for safeguarding all government property provided for contractor use. BASE ACCESS, PASSES AND IDENTIFICATION: The contractor shall obtain personal identification photo passes for all employees and vehicle passes for all contractor and personal vehicles requiring entry onto Hurlburt Field AFB from Security Forces Pass and Registration, for the duration of the contract. Vehicle registration, proof of insurance and a valid driver?s license must be presented for all vehicles to be registered. Contractor shall comply with the requirements in AFFARS 5352.242-9000 Contractor Access to Air Force Installations. The contractor must have a National Agency Background Check accomplished on activation of the contract. Contractor personnel shall successfully complete, as a minimum, a National Agency Check (NAC), before operating government furnished workstations that have access to Air Force e-mail systems. The government at no additional cost to the contractor shall submit these investigations. The contractor shall comply with the DoD 5200.2-R, Personnel Security Program, and AFI 33-119, Electronic Mail (E-Mail) Management and Use, requirements. Continued employment is contingent upon successful completion of the (NAC). Quotes may be transmitted by e-mail or faxed to (850) 884-3264.
