Description

The 115 Fighter Wing is soliciting quotes for two (2) truck hoists, to be delivered to the Wisconsin Air National Guard, Truax Field, Madison WI 53704. This is a combined synopsis/solicitation for commercial items prepared in accordance with the for mat in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This requirement is a 100% SMALL B USINESS set-aside acquisition. Issued as a Request for Quotation, proposals are being requested and a written solicitation will not be issued. The NAICS code is 423120 and the Standard Industrial Classification (SIC) 5013 applies to the procurement. The contractor shall provide the following items OR EQUAL; the Ammco specification is used as a baseline only; all OR EQUALS will be considered, final approval concerning OR EQUAL will be made by the Contracting Officer; CLIN 0001  (2) Ammco Lifts, Par t Number 4S21SSR1, standard height (144 tall), 220v 1 phase; to include truck adapters CLIN 0002: Contractor installation. The offeror shall include descriptive literature such as specifications, brochures or other manufacturing information that fully d escribes the item being offered. The item is to be delivered FOB Destination within 90 days ARO to the following address: 115 MXS, 3110 Mitchell Street, Building 1000, Madison WI 53704. The following provisions are included in this acquisition: FAR 52.2 12-1, Instruction to Offerors-Commercial Items, Commercial (Jan 2004). The Government will make an award to the responsible offeror whose offer conforming to the solicitation is the lowest price. Offerors shall include with their quotes a completed signe d copy of 52.212-3, Offeror Representations and Certifications-Commercial Items (Jan 2004).
