Description: The Phoenix Area Indian Health Service is soliciting proposals for Orthopedic Surgeon Services to provide on-site orthopedic surgeon services for the Whiteriver Indian Hospital, 200 W. Hospital Drive, Whiteriver, AZ 85941. Services are required three (3) days a week to the Whiteriver Indian Hospital. This is a non-personal services contract for an estimated 2,000 hours per year. Contract for a base year with four (4) 1-year option years. Closing date for receipt of proposals is September 8, 2006, 4:30 P.M. (MST). Requests for RFP 970 may be obtained by written request only.
