- Description
- The VA Healthcare Network Upstate New York (VISN 2) desires to enter into a multiple award contract with Joint Commission on Accreditation of Healthcare Organizations (JCAHO) Certified Contractors to provide storage, delivery, set-up, instruction-in-use, and/or pick-up of Government purchased items, in accordance with all terms, conditions, provisions, specifications and schedules herein. Deliveries and/or pick-ups to be made at such times and in such quantities as specified by the VA Medical Center Contracting Officer or his/her designated representative (COTR). JCAHO Standards for the Accreditation of Home Care, Section 1, Chapters I through VII, and Section 2, Chapters VIII through IX are applicable to service provided under this contract. JCAHO Standards can be found at http://www.jcaho.org. The VA intends to award approximately 3-5 Firm Fixed-Price, Indefinite Quantity, Indefinite Delivery contracts pursuant to this solicitation for commercial items, however the VA does reserve the right to award to one contractor, if it is determined to be in the best interests of the Government. The VA shall determine (based on certification, training, past performance, and price proposals) the individual proposals judged to be most advantageous to the Government. Offerors should submit a price for each item for each major site that they are interested in providing services to. Proposals may be submitted for one or all of the major sites. This is a Service Disabled Veteran Owned Small Business Set-Aside Cascaded Procurement. ALL responsible small business concerns are encouraged to submit proposals. VA intends to evaluate offers and make award as indicated. Service Disabled Veteran Owned Small Business Participation. In accordance with Federal Acquisition Regulation Part 19, any award for the storage and delivery of durable medical equipment from this solicitation will be made on a competitive basis to a responsible Service Disabled Veteran Owned Small Business (SDVOSB) concern that provides fair and reasonable proposal in accordance with all of the solicitation requirements, provided that at least one (1) responsive and acceptable offer is received. If a minimum of one (1) offer from an eligible, responsive, and responsible SDVOSB concern is not received, and/or if the SDVOSB offers are found otherwise ineligible for award, other offers will be evaluated in the same manner for the following socio-economic groups as prioritized:
