- Solicitation Number: W67K2S-6068-A500 Posted Date: 08/21/2006 Response Date: 09/05/2006 Set-Aside: Total Small Business Contracting Office Address: USPFO for Utah, 12953 South Minuteman Drive, Draper, UT 84020 Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; p roposals are being requested and a written solicitation will not be issued. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-11. The solicitation number is W67K2S-6068-A500 and is issued as a Request for Quotation (RFQ). Quotations shall be received in the contracting office by 4:30 p.m. (MST), September 5, 2006. The RFQ shall result in a Firm-Fixed Price Contract. This solicitation is 100% small business set-aside, with a small business size standard of $9M. The North American Industry Classification System (NAICS) Code for this acquisition is 621999. SPECIFICATIONS: The USPFO for Utah has a requirement to contract for Health screen tests for approximately 160 employees for the Utah National Guard. Testing is scheduled for September 12 and 13, 2006, at the headquarters buildings, 12953 South Minuteman Drive, Draper, Utah and on September 14, 2006 at the AASF Facility, 7602 South 4470 West, West Jordan, Utah. Hours for testing, is scheduled from 7:00 a.m. to 4:30 p.m. The 160 employees will be distributed between the two locations. Health screen tests include hearing tests, test interpretation, explanation of individuals results, a copy of the results for each employee, hearing conservation training. Medical clearance for respirator use, full respiratory medical history per current OSHA respiratory p rotection standard, health screening services of blood pressure, heart rate, height, weight, vision testing, pulmonary function test, review of records with a professional licensed health care provided, and physician review of records and results with medi cal clearance certification. Shall be willing to work with the occupational health nurse to ensure the delivered product is in a format compatible with the Defense Occupational Health Readiness System Hearing Conservation (DOHRS-HC) so data can be downl oaded and forwarded to Ft. Detrick. Health screen tests are to be conducted on site using a mobile unit. Contractor is responsible to furnish all personnel and equipment necessary to provide testing. The Government will provide suitable space for parkin g, electrical and water connections for the mobile testing unit. EVALUATION FACTORS: In accordance with FAR52.212-2 Evaluation of Commercial Items (Jan 1999) (a), The Government intends to make an award to the Contractor who submits the lowest, technically responsive proposal, and is deemed acceptable and responsible by the Contracting Officer. INSTRUCTIONS: Offers shall include (1) per-person cost for testing with terms fob destination. Offerors shall complete electronic annual representations and certifications at http://orca.bpn.gov in conjunction with required registration in the Central Con tractor Registration (CCR) database. In accordance with FAR 52.212-1(k), prospective awardee shall be registered and active in the CCR database prior to award. If the offeror does not become registered in the CCR database by the due date/time of quote, t he Contracting Officer will proceed to award to the next otherwise successful registered offeror.
