Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the information in FAR 12.6 using Simplified Acquisition Procedures for commercial items found at FAR 13 as supplemented with the additional information included in this notice. This announcement constitutes the solicitation; a written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. Solicitation N00259-06-T-0184 applies and electronic copy can also be accessed at http://www.neco.navy.mil. This acquisition is set-aside for small business. The agency need is pastoral care services. The NAICS is 813110. The period of performance will begin Clin 0001: 10/01/06 thru 12/30/2006 and Clin 0002 1/01/2007 thru 09/30/2006. Quoters must be registered in the central contractor registration; website is www.ccr.gov to be considered for award. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular in fac 2005-11 and DFAR Change Notices effective through 6/21/05. OFFERORS ARE REQUIRED TO COMPLETE AND INCLUDE A COPY OF THE FOLLOWING PROVISIONS WITH THEIR PROPOSALS: FAR 52.212-3, Offeror Representation and Certifications ? Commercial Items. 52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (1) past performance. Provide references, specifically addressing: pastoral care services in the last 3 years. (2) technical capability. Defined as an assessment of the prospective contractor's key management personnel to determine if they have the basic knowledge, experience, and understanding of the requirements necessary to produce the required product or provide the required service, must specifically address a management plan in adequately staffing the task order effort with individuals who meet the qualifying knowledge, skills, abilities, experience for providing pastoral care services and (3) price. Contractors who do not meet the first 2 factors will not undergo a price evaluation. Past Performance and Technical Capability will have more importance than cost or price. Prior to Contract Award, the government reserves the right to review the resumes of contractor employees performing under the contract solely for the purpose of ascertaining their qualifications relative to the personnel qualifications terms of the contract. Accordingly, the contractor shall furnish such resumes and all professional requirements as stated in the Statement of Work (SOW) to the Purchasing Agent along with their quotes. The Senior Roman Catholic Chaplain conjointly with at lest one other NMCSD Staff Chaplain will interview each applicant as stated in the other considerations/requirements, letter in the SOW. Description of Supplies/Services: CLIN 0001 Non-Personal Services for Pastoral Care for daytime catholic priest IAW the statement of work. STATEMENT OF WORK- NONPERSONAL ROMAN CATHOLIC PRIEST SERVICE 1. CONTRACTOR REQUIREMENTS a. The contactor shall provide sacramental and pastoral care and counseling to staff, patients and their families, prepare and maintain records, be available to celebrate daily Mass, participate in meetings required by the Director of Pastoral Care Service, and respond to other Catholic needs as they may arise. b. During the term of this contract, the contractor agrees to perform on behalf of the U.S. Government, the duties of a Roman Catholic priest for the spiritual care of active duty military personnel, their dependents, eligible Navy civilian employees, and other beneficiaries, per the terms and conditions of the contract. b. As an independent contractor, all medical, dental, and professional liability insurance is the responsibility of the contractor. 3. REFERENCES The following documents and data sources are expected to be needed by the contractor in assisting the Navy Catholic Chaplain to provide pastoral and sacramental care, and will be provided upon request. This information will be provided by the medical center?s Pastoral Care Service staff: a. Demographic information on the beneficiary population served. b. Current data on patient preferences as determined by survey, correspondence, and patient relations statistics. c. Information regarding non-medical issues such as facilities, parking, support services, staffing, watch bills, and other amenities. d. Results of inspection/surveys by internal and external agencies. e. Assessment of employee moral and job satisfaction as determined by annual survey of entire staff. 4. PROPERTY a. Contractor- Furnished Property. The contractor shall finish any computer hardware and software necessary for use by the contractor. b. Government- Furnished Property. The Government will provide office space, office furniture, telephone/fax service, and the use of a copier. The Government will allow unlimited telephone usage, both local and long distance, in support of the Pastoral Care Service program. The Government will provide the referenced information specified in Section 2. The Pastoral Care Service staff will coordinate the retrieval of information needed by the contractor and serve as the interface. The Government will provide resources for the administration and celebration of sacraments and other professional resources deemed necessary by the Director of Pastoral Care Services. The Government will provide office staff for administrative support. 5. OTHER CONSIDERATIONS/REQUIREMENTS a. Place of Performance. Work shall be performed on site. Meetings between the contactor and the Director of Pastoral Care Service, or his representative, shall be performed on site. b. The contractor shall work 25 hours per week, Monday through Friday, 52 weeks per year. Working hours will normally be during the hours of 0730-1230. Flex hours may be employed at the consent of the contractor of the Director for Pastoral Care Service. When the Navy Catholic Chaplain is on annual leave, TAD, or other legitimate absence, the contractor will be responsible to provide coverage for the Daily Masses during those times. Four hours of credit will be given to each Sunday Mass celebrated. Leave and holidays will be approved by the Director, Pastoral Care Service in accordance with NMCSD policy. c. Communication. The contractor is authorized to meet with Department Heads, Directors, supervisors, and other staff personnel as well as patients and their families in the performance of pastoral responsibilities. The contractor shall pursue all administrative matters through the Director Pastoral Care Service. The Director of Pastoral Care Service will provide a written performance evaluation to the contractor mid-year and at the close of the fiscal year. d. Personal Qualifications. The contractor shall staff the contract with a validity ordained Roman Catholic priest in good standing with his diocese or religious order and endorsed by the Archdiocese for Military Services. The Ecclesiastical Endorsements shall be submitted in writing. The Catholic priest shall demonstrate reliability, good relational skills and the ability to serve on an ecumenical ministry team. The contracted priest shall comply with Government policies and regulations in regard to identification badges, parking, and professional decorum. e. Primary Point of Contact. The senior Navy Catholic Chaplain (Roman Catholic Program Coordinator) on staff at NMCSD is the primary point of contact for the contracted priest. f. No alien shall be allowed to perform under this contract without an Alien Registration Card (?green card?) and/or in violation of the Immigration Laws of the United States. g. The contractor shall be neat, clean, well groomed, and in appropriate clergy clothing (shirt with Roman collar) when in patient care and public areas. The contractor shall display an identification badge on the right breast of his/her clothing, which includes the contractor?s full name, professional statues, and expiration date. h. The contractor shall become acquainted with and obey all pertinent Command, Navy, and DOD regulations, shall perform in a manner which precludes any waste of utilities and shall not use Government telephones (of other Government equipment, e.g. loaner pagers) for personal business. All motor vehicles operated on this installation by the contractor shall be registered with base security and operated according to applicable directive. Eating and smoking by the contractor are prohibited in patient care areas and restricted to designated areas. i. The contractor shall be able to read, write, speak, and understand the English language fluently. j. Unplanned absences due to illness or other incapacitation of the contractor will be allowed up to a maximum of 15 days per contract year. The contractor shall advise the COR (senior Navy Catholic Chaplain, the Roman Catholic Program Coordinator) of such absences. If the contractor is absent for four or more weeks due to illness, he shall provide a written documentation from a qualified health care provider that he is free from any communicable disease. The Government reserves the right to examine or reexamine any such contractor who meets this criterion. k. The contractor shall provide a competent substitute priest during any absent period. He must be approved by the Director of Pastoral Care Service and the senior Navy Catholic Chaplain. The substitute priest will provide the same pastoral care during the absence of the contractor. l. A physical examination certification, which states the date on which the physical examination was conducted, the name of the doctor who performed the examination, and a statement concerning the physical health of the contractor. The certification must contain the following statement: ?(Name of contractor) is suffering from no physical disability or medical condition which would restrict or preclude his/her from providing services as a Roman Catholic Priest. (Name of contractor) is suffering from no contagious diseases to include but not limited to AIDS, tuberculosis, hepatitis, and venereal disease, and is not positive for HIV antibodies.? m. A resume must be submitted in advance for technical review including total annual salary based upon hours to be worked under contract. If this information is proprietary to subcontractors, it may be provided under separated cover; however, it must be easily identifiable and readily combined with the rest of the proposal. Failure to provide this information may impact the Government?s evaluation of the contractor?s proposal. n. Interview. The Senior Roman Catholic Chaplain conjointly with at least on other NMCSD staff chaplain will make and interview each applicant. The interview will reflect a review of the overall package with emphasis on the following: a. Overall professional training and experience of the candidate as a hospital chaplain. b. Cost of the contract. c. Military experience and/or understanding of military methods by the candidate. d. Flexibility of the candidate in meeting the needs of the hospital patients and staff within the parameters of the contract. e. Perceived ability of the candidate to do sustained and intense hospital ministry. f. Perception of the candidate as a team player who can edify and be edified by the Pastoral Care Service team. g. The government intends to make a single firm fixed contract award to the responsible offeror whose offer is the most advantageous to the government considering price and price-related factors.
