Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number W81K00-06-T-0358 is being issued as a Request for Quotation. The RFQ and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-12. The procurement is set aside 100% for small businesses, NAICS code is 339113, small business standard size is 500 employees. Requirement is for 1 Each OSI Imaging Table System or equivalent. Systems includes 1 each Advanced Control Retractable Ba se for the Modular Table System, Model No. 5803; 1 each Radiolucent Imaging Top, 500 pound capacity with TEMPUR-MED table pad No. 5927; 2 each Pivoting Arm Board with TEMPUR-MED Pad No. 5356; 2 each Side Rail Adapters for Orthopedic and Imaging Top No. 585 5-830; 1 each A/P Cassette Tray for Imaging Top No. 5839; 1 each Shipping; 1 each Installation to include uncrating; 1 Day Staff Service; 3 each Operators Manual; 2 each Maintenance Manuals; Standard Commercial Warranty on parts and labor. Provide produ ct specifications with quote. Items to be shipped to and installed at Brooke Army Medical, Fort Sam Houston Texas 78234-6200 within sixty days after receipt of order.
