Description

This is a combined solictation/synopsis is for commercial item from Steris Corporation CLIN 0001 #LV05 HARMONY LC DUAL 500 PKG NON CAMERA READY 2 EA. CLIN 0002 #LY01 CONTROL PANEL ROUGH-IN BOX WITH MUD RING 2 EA. CLIN 0003 #LV00006 SUSPENSION TUBE 9INCH/225MM 2 EA. CLIN 0004 # LV00003 LOW CEILING SENCONDARY CANOPY KIT 2 EA. CLIN 0005 #FIELD ENGINEERING SERVICES(INSTALLATION COST). Prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Quotes are being requested and a written solicitation will not be issued. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-12. The associated North American Industry Classification System (NAICS) Code is 339112. The US Naval Hospital intends to award an firm fixed-price award for following medical item. The evaluation factors used to evaluate offers are technical capability of the item offered to meet the Government requirement, price and delivery. Delivery and acceptance point shall be DDJC Warehouse 30 CCP 25600 S. Chrisman Road Tracy, CA 95304. Contractor will be required to deliver by Sept 22, 2006 and call the government POC prior to expected delivery schedule for required item. This solicitation incorporates the following FAR Provisions: 52.204-6 Data Universal Numbering System (DUNS) Number; 52.212-1 Instruction to Offerors -- Commercial Items; 52.212-4 Contract Terms and Conditions -- Commercial Items; and 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders --Commercial Items. All responsible sources must submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications, Commercial Items with its offer. All quotes need to be submitted NLT 25 Aug 06 1:00 p.m. JST (Japan Standard Time). The anticipated award date is 30 Aug 2006.
