- The GLAC (Great Lakes Acquisition Center) is soliciting quotations for a contractor to provide Radiopharmaceutical Services for beneficiaries for the following VISN 12 Medical Centers: Edward Hines VA Hospital, Fifth Avenue & Roosevelt Road, Hines, Illinois 60141; Jesse Brown VA Medical Center, 820 South Damen Avenue, Chicago, Illinois 60612; and for Clement J. Zablocki VA Medical Center, 5000 West National Avenue, Milwaukee, Wisconsin 53295 for the period of October 1, 2006 through September 30, 2007, with up to one one-year option to extend at the Government?s discretion in accordance with all specifications, work statement, terms and conditions contained therein. All qualified offerors/firms who believe they can satisfy the solicitation requirements are invited and encouraged to provide their proposal. Offers shall be evaluated for fair and reasonable pricing, technical competency/capability to perform services and satisfactory past performance record. The Government anticipates award of a commercial item contract to the responsible offeror whose offer provides the best value to the Government. The VA reserves the right to select the service providers from different contractors resulting in multiple awards. This is a Firm Fixed Price contract and this acquisition is a Full and Open Market buy. Offerors are reminded to complete all sections of the solicitation, including the Offeror Representations and Certifications-Commercial Items. Requests for hard copy solicitation must be in writing to the Contracting Officer at the GLAC and accompanied by a check for $5.00 made out to the Department of Veterans Affairs. No telephone requests will be accepted. Any amendments issued to this solicitation will also be available at the URL address. Offerors are advised that they are responsible for obtaining amendments.
