Description
This is a combined synopsis/solicitation for commercial items prepared IAW the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being r equested and a written solicitation will not be issued. This synopsis/solicitation is issued as a Request for Proposal for an Indefinite Delivery Indefinite Quantity type contract for Mobile Dental Services for the Kansas Army National Guard. These mobil e dental services must include a fully functional mobile system including mobile equipment, staff, patient education, statistical analysis and reports, emergency resources, and 100% DENCLASS/MEDPROS data entry. The mobile system must be a self-contained p ractice that offers patient privacy and confidentiality, without risk of an inadvertent HIPPA communication violation. The system must be fully digital and require no chemicals needed for the development of film. The mobile system must be fully OSHA comp liant, complete with medical waste disposal, proper ventilation, and have fully lead-lined walls around each operatory compartment. Contractor shall be licensed on any services rendered and equipment operated during mission. The requirements of the Den tal Screening Missions include: 1.) Clinical evaluation by a licensed dentist of the oral cavity using a visual, clinical exam supported by bitewings taken by dental technicians and the panoramic x-ray taken by dental technicians if clinically indicated in the dental record. 2.) Each exam will be entered into the DENCLASS reporting system within 5 days after the individual Dental Screening Mission and annotated on the SF 603 (Health Record-Dental) and the SF 5570 (Health Questionnaire for Dental Treatme nt) of the Soldiers dental records. 3.) Personnel performing the dental screening examination have a moral and ethical obligation to inform the Soldier if they observe or are apprised of any sign or symptom for which the Soldier should obtain further ev aluation or care. 4.) A dental screening examination identified deficiencies but does not include the dental treatment required to restore those deficiencies. 5.) Soldier Readiness Processing (SRP) for Mobilizations will include treatments for those id entified in Class III. Contractor will provide all items and licensed personnel needed to perform the Dental Readiness requirements, any employee benefits, employee accommodations, food and travel expenses, Vacation days, paid holidays, and Sick Leave Pla n. Contract shall be awarded for one year with three additional option years. Award will be an Indefinite Delivery Indefinite Quantity Contract with a minimum guaranteed order of $10,000.00. Cumulative Total of Delivery Orders shall not exceed $500,000. 00. Award will be made to the lowest priced technically acceptable proposal. Technically acceptable will be determined based on the contractor having the necessary equipment, personnel, and also having satisfactory prior experience in providing Mobile De ntal Services to the Military on at least 3 prior contracts. Proposal must include names and points of contacts of other Military customers who they have performed this or similar work for in the past five (5) years. Pricing should be on a per soldier ba sis and should include all staff, expenses, equipment, and services. The government will pay for actual number of exams performed based on the per soldier price. Lowest price will be evaluated by the lowest overall average per soldier price of the base y ear and all option years. In order to receive an award of this contract, you must be registered in the Central Contractor Registration database at www.ccr.gov. All responsible sources may submit an offer which shall be considered by the Agency. This sol icitation W912JC-06-R-4021 is issued as an RFP. This is a solicitation document and incorporates provisions that are in effect through Federal Acquisition Circu lars. The provision at FAR 52.212-3, Offeror Representations and Certifications (7/2002) applies to this acquisition. A completed-signed copy of this provision shall be submitted with any proposal.
