- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes ar e being requested and a written solicitation will not be issued. The solicitation number is W91YTV-06-T-0242. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-05. The North American Industry Classification System Code (NAICS) for this procurement is: 339113; the Standard Industrial Classification Code is 3841; and the small business size standard is 500 employees . Th is requirement is for 100% Small Business Setaside. Martin Army Community Hospital, Bldg. 9222, Santa Fe Road, Fort Benning, GA 31905-6006. has a requirement for the following: CHARLOTTE SHOULDER SYSTEM, UNIT PRICE $__________, TOTAL CLIN $__________. The delivery terms will be for all material to be delivered on or about 31 October 2006 or sooner. Please include shipping and handling cost if applicable. The government anticipates awarding a single award for this requirement.
