- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, RMK0E060085, is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 2001-03. The North American Industry classification (NAICS) code is 624310 and the business size maximum is $5.0 Million. The proposed contract is a Total Small Business Set-Aside. All small business shall provide a breakdown of their costs to demonstrate that at least 51% of the cost of contract performance incurred for travel, supplies, training materials, etc. shall be performed and incurred by the small business (prime). All small businesses shall comply with FAR 52.29-14 Limitations on Subcontracting. All responsible small business sources may submit a quote, which will be considered by the Government. This office is requesting quotes on the following item: 001: Provide all travel, supplies, materials and personnel to provide psychological evaluator services to the BIA, BIE, Pine Ridge School; Pine Ridge, SD in accordance with the Statement of Work. Please provide your cost to provide 1. 72 days of counseling services. 2. 2 hrs per day for 8 days per month x 9 months of consultation services. 3. Travel Costs during the base year (date of award to June 30, 2007) and four option year periods (each option year will begin in July 1 and end June 30). The contract shall be awarded as a firm fixed price contract with a base year and four (4) option year periods. STATEMENT OF WORK. Therapeutic Counseling Services. Kayenta Community School, North Highway 163, Kayenta, Arizona is in need of a contractor to provide Therapeutic counseling services on-site beginning (date of contract approval thru June 30, 2007). The contractor will provide one-on-one and small group counseling for 12 (twelve) students two times per month. These students currently have Individual Education Plans citing counseling as a related service needed for their own academic well-being and safety. The multidisciplinary team feels that these children need regularly scheduled counseling in such areas as anger management, grief, behavioral, and social issues. The contractor will conduct further assessment on identified students, amend IEP's and develop a visitation schedule. The contractor will confirm with P.L. 105.17 and 34 CFR, Part B; 25 CFR 45:1-80 and Section 504 of the Rehabilitation Act of 1973: 29 USC 794. The Therapeutic Counseling Service will include: 1. The administration of assessment chosen and/or approved by the school's Special Education Department. 2. Scoring and short summaries of all assessments administered. 3. The interpretation of all assessment results. 4. Documentation that parents and staff receive a copy of the individualized summary. 5. Participation in MDT meeting to interpret results and respond to questions. 6. Provide therapeutic counseling in individual, small group, large group settings, twice a month. Schedule on the same days, provide IEP reviews and input on student's progress and other related counseling service needs. 7. Provide Kayenta Community School Special education Department and Western Navajo Agency, Special Education with a copy of the service provider's state certification. The provider must have a security background investigation clearance. 8. Attendance at the Western Navajo Agency, Special Education Provider's meeting (schedule date will be announced for November 2006). The contractor will not be allowed to add service days and additional lodging without the modification approval from the contracting officer. OTHER REQUIREMENTS. Contractor must provide name of the service provider who will be assigned to Kayenta Community School. The service provider shall have a favorable background investigation on file with the BIA, BIE, Security Office. Call 505-563-5287 for information and forms. Qualifications of Expert Provider. Knowledge, skills and abilities required of expert provider: 1. Experience working with BIA, BIE schools in the area of Therapeutic Counseling Services. The Offeror shall provide the lump sum cost for the base and four option year periods with a breakdown of their cost to provide this service in accordance with the four areas in the Breakdown of Costs Section ; and, provide a copy of their credentials, including but not limited the college diploma. The offeror shall have a favorable screening (applicant and suitability) determination must be made by the Bureau of Indian Affairs (BIA) Office of Indian Education Programs (OIEP), Personnel Security and Suitability Program in accordance with the Background Investigations Clause.
