- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, RMK0E060084, is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 2001-03. The North American Industry classification (NAICS) code is 624310 and the business size maximum is $5.0 Million. The proposed contract is a Total Small Business Set-Aside. All small business shall provide a breakdown of their costs to demonstrate that at least 51% of the cost of contract performance incurred for travel, supplies, training materials, etc. shall be performed and incurred by the small business (prime). All small businesses shall comply with FAR 52.29-14 Limitations on Subcontracting. All responsible small business sources may submit a quote, which will be considered by the Government. This office is requesting quotes on the following item: 001: Provide all travel, supplies, materials and personnel to provide psychological evaluator services to the BIA, BIE, Pine Ridge School; Pine Ridge, SD in accordance with the Statement of Work. Please provide your cost to provide 50 days of Psycho-education for students and 15 Clinical Evaluations for students during the base year (date of award to June 30, 2007) and four option year periods (each option year will begin in July 1 and end June 30). STATEMENT OF WORK. Objective: To provide psychological services to the Pine Ridge School Special Education Program in accordance with IDEIA regulations for special needs students. I am writing on behalf of the Pine Ridge School to provide the requested information regarding the job responsibilities and duties of the requested psychological evaluator for the Special Education Program for the 2006-2007 school year. The psychological evaluator will be primarily responsible for evaluating students for determining special education eligibility for the special education program, and collaborating with the special education and regular education personnel on implementing a program/plan for the qualifying students. The psychological evaluator will provide psycho-educational evaluations (intelligence/IQ scores) to assist the program in determining whether students qualify and/or continue to qualify for special education services to address their individual learning concerns. The psychological evaluator will provide clinical evaluations (social-emotional-behavioral results) to assist the program in determining whether students will require special education services to address their social-emotional-behavioral concerns. The psychological evaluator will provide attention-deficit hyperactivity disorder (ADHD) screenings to assist the program n determining whether students will require special education services and/or medical referrals to address their inattention and hyperactivity concerns. The psychological evaluator will provide evaluation reports that will interpret (i.e. diagnose) individual test results to assist the program in determining individual programs/plans for students in the school program. The psychological evaluator will consult with the special education and regular education programs to interpret evaluation/screening results to assist them in devising an appropriate plan (i.e. Individualized Education Program/Plan) to address the student's education needs in the school system. The period of duty will be from Date of Award through June 30, 2007. The days of psychological services will be dependent on the need (based on student evaluations) of the psychological evaluator and agreement with the special education program based on the school schedule for the 2006-2007 school year. OTHER REQUIREMENTS. Contractor must provide name of the service provider who will be assigned to Pine Ridge School. The service provider shall have a favorable background investigation on file with the BIA, BIE, Security Office. Call 505-563-5287 for information and forms. Qualifications of Expert Provider. Knowledge, skills and abilities required of expert provider: 1. Experience working with BIA, BIE schools in the area of psychological services. The Offeror shall provide the lump sum cost for the base and four option year periods with a breakdown of their cost to provide this service in accordance with the four areas in the Breakdown of Costs Section ; and, provide a copy of their credentials, including but not limited the college diploma. The offeror shall have a favorable screening (applicant and suitability) determination must be made by the Bureau of Indian Affairs (BIA) Office of Indian Education Programs (OIEP), Personnel Security and Suitability Program in accordance with the Background Investigations Clause. The Offeror shall provide the lump sum cost with a breakdown of their cost to provide this service and respond to the Evaluation Factors delineated at the end of this synopsis.
