Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested and a written solicitation will not be issued. Solicitation W91YTV-06-T-0251 is being issued as a Request for Quotation. The solicitation document and incorporated clauses are those in effect through Federal Acquisition Circular 2005 -12. This acquisition is a 100% set-aside for small business concerns, associated NAICS code is 339112 and the size standard is 500 employees. The government intends to purchase a Glaucoma Diagnostic System using Optical Coherence Topography for Winn Arm y Community Hospital, Fort Stewart, GA. CLIN 0001, Glaucoma Diagnostic System - OCT, 1 each, Description: Glaucoma Diagnostic System, utilizing Optical Coherence Tomography (OCT) to diagnose Glaucoma damage. System must be capable of assessing Macula thi ckness and other pathologies, provide analysis of the Retinal Nerve Fiber Layer (RFNL), and perform Optic Nerve Head Analysis. An acceptable system will include a printer and come with stand or workstation that is easily moved. Two sets of all operator a nd service manuals shall accompany equipment. Zeiss Meditec's Stratus OCT with NDB Printer and Table - Brand Name or Equal. Terms are FOB Destination--Winn Army Community Hospital, Building P302, 1061 Harmon Ave, Fort Stewart, GA, 31314-5611. Required de livery date is September 29, 2006. The provisions of FAR 52.212-1, Instructions to Offerors-Commercial applies to this acquisition without addenda. The provisions of FAR clause 52.212-4, Contract Terms and Conditions-Commercial Items, apply to this acquis ition with addenda that direct the use of Wide Area Workflow to submit electronic invoices if available.
