- The U.S. Department of Veterans Affairs, Carl T Hayden VA Medical Center, 650 E Indian School Road, Phoenix, Arizona 85012-1892 is seeking chiropractic services for eligible Veterans at the Phoenix Carl T Hayden VA Medical Center, for a base period of October 1, 2006 through September 30, 2007 with three option years ending September 30, 2010, per the performance requirements in the solicitation VA-644-06-RP-0004. The North American Industry Classification System Number (NAICS) for the services is 621310 and the Business Size Standard is $6.5M. This solicitation is 100% set-aside for Small Business. The solicitation will be made available on or about August 7, 2006, with a due date of September 7, 2006, by 3:00PM US/Arizona MST.
