Description
The purpose of this amendment is to incorporate: Questions and Answers How many ambulances are required? One ambulance is required to respond to be posted at bldg 700. However a backup plan is needed if two or more calls are simultaneous. Are their housing quarters for Contractor employees outside the office space that will be provided? No Is this a personal services contract? No How many CLINs are in the bid process? 4 Clins a basic and 4 option years. Will be priced by the month with yearly price and options. What radio communication system is required, if any? The 78th Medical Group will provide two hand held radios for access to Robins’ AFB bands. Two methods: - The ambulance must have the state band radio. - Contact via cell phone According to 1.2.7, what are considered “non-duty” hours? Per 1.0, Duty hours are defined as 0600-0600. This covers a 24 hour period so we are unsure of the “non-duty hours” statement. Please clarify. Non duty hours 1700-0700. According to 1.2.8, what does the semi-annual training consists of as specified in the SOW? The required training is decompression sickness and specified training on any equipment unique to our flight medicine clinic. Please provide details on the mutual aid assistance regarding HMC, Warner Robins, and Houston County 911. In the event of a mass casualty basically all agencies will join forces to ensure prompt medical care When can a site visit be arranged? Yes Are ambulances required to be licensed with the State of Georgia? Yes, also paramedic and EMT require GA licensure/certification Can we bill for services due to MOAs with area hospitals, if applicable? yes There is a MOA throught the Hospitals and will post Contractor shall provide emergency patient (Ambulance Service) transport services at Robins AFB GA and all associated housing areas. In accordance with the attached Performance Work Statement. 12 month service with 4 annual option years. Period of Performance: 18 Sept 2006 - 17 sept 2006 with 4 annual option years. This is a combined synopsis/solicitation for commercial items prepared IAW the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being required and a written solicitation will not be issued. The following clauses are applicable to subject solicitation: FAR 52.212-1 (Instructions to Offerors), 52.212-2 (Evaluation - Commercial Items), 52.212-3 (Offeror Representations and Certifications - Commercial Items), 52.212-4 (Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items). IAW clause 52.212-2, the evaluation factors shall be: Price and Price related factors, Conformance to performance specifications (SOW). This solicitation incorporates provisions and clauses that are in effect through Federal Acquisition Circular Current to: FAC 2005-11, Effective 05 Jul 2006. NAICS Code 621910. Place of erformance/FOB Robins AFB, GA. Service shall begin on 18 Sept 2006. Convict labor 52.222-21, Prohibition of Sergregated Facilities 52.222-26, Equal Opportunity 52.222-41, Service Contract Act of 1965, As amended 52.222-42, Statement Of Equivalent Rates for Federal Hires 52.232-33, Payment by Electronic Funds Transfer-Central Contractor Registration 52.233-3, Contractor Identification Wage Determination 94-2139 Rev.32 dated 05/24/2006 applies to this solicitation.
