Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotes under the number RFQ21602-0024-06. This solicitation document, incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-12. This action is a Total Small Business Set Aside. The North American Industrial Classification System (NAICS) Code is 621399 - Offices of All Other Miscellaneous Health Practitioners and the Small Business Size Standard is $6.5 Million. GENERAL REQUIREMENTS: The Federal Bureau of Prisons, Field Acquisition Office, Grand Prairie, Texas, intends to award a indefinite delivery/requirements type contract with firm-fixed pricing for the provision of Radiologic Technologist (X-ray Tech) services which meets the requirements outlined in this solicitation. PLACE OF PERFORMANCE: Contract services will be performed on-site in the Health Services Department at the Federal Correctional Institution (FCI) Elkton, located at 8730 Scroggs Road, Elkton, Ohio. REQUIRED SERVICES: The contractor shall be capable of providing the services of a Radiologic Technologist. The contractor shall be responsible for, but not limited to, the following: A) The contractor is required to perform all X-Ray examinations as written by the health care providers of FCI/FSL Elkton (i.e., Physician, Dentist, Physician Assistant, and Nurse Practitioner). B) Knowledge and use of GE Advantx x-ray generating equipment. C) Knowledge and use of AGFA CR digital radiological equipment. D) Knowledge and use of Fuji image copier for digital radiological equipment. E) Alter and QC digital images and send digitally to a MedWeb internet system. F) Maintain exposure log books. G) Maintain paper filing system for patient encounters and final reports. This will include entering the patient data into the computer. H) X-ray reports will be retrieved from the computer and delivered to the Medical Records staff. I) The contractor will have regular contact with the inmate population in a patient/medical provider role. J) Contractor will be responsible for filing of all X-ray films and reports. K) Contractor will purge the files and prepare for mailing all x-ray films for inmates transferred to other facilities. L) Must be knowledgeable in the use of computer programs and e-mail. All duties shall be performed in accordance with standards and methods generally accepted within the field of Radiology and in compliance with all Federal, State policy and regulations. The contractor shall adhere to all standards and regulations associated with the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and American Correctional Association Foundation/Core Standards for Adult Correctional Institutions. The Contractor will be responsible for following directives and complying with Bureau of Prisons (BOP) Health Services Manual, FCI Elkton institution mandates and the Health Service unit's policies in the management of all patient encounters. The contractor's performance shall be monitored by the Contracting Officers Technical Representative (COTR). CERTIFICATIONS AND REQUIREMENTS: The contractor must be registered and licensed with the proper authorities to provide requested services as prescribed in this solicitation with-in the state of Ohio. The contractor must have a valid national (ARRT) American Registry of Radiographic Technologist certification and a valid license issued by the Ohio Department of Health. Contract staff providing service must be experienced and certified to perform requested studies and tests. Have a minimum of one year of experience in X-ray Technology (experience in a correctional setting is preferred, but not required). The contractor must also be certified in Basic Cardiac Life Support and provide a copy of his/her certification. SCHEDULE OF PERFORMANCE: Services will be required on an as needed basis. There is no minimum number of x-ray tests completed each year. The number of tests completed will be determined by the number of inmates which require x-ray examinations. The contractor will perform between ten (10) to thirty (30) sessions per week. The exact days and performance start time will be mutually agreed upon between the contractor and the COTR. Work hours will be scheduled between the hours of 6:00am and 4:00pm, Monday thru Friday. The contractor will be required to visit the institution up to five (5) times per week. All Federal Holidays and any other day authorized by the Executive Office will be excluded. Any changes to this schedule shall be coordinated through the Health Services Administrator, COTR and/or the Contracting Officer, and is not expected to exceed 30 hours per week. The schedule is subject to change based on the variable needs of the institution and the unpredictable nature of the correctional environment. Upon agreement, any changes to the schedule must be coordinated with the COTR assigned under the contract. The contractor will be available for emergencies as necessary. Due to the nature of the correctional setting, delays in performance of work may be expected. Such delays may include but are not limited to inclement weather, electrical or mechanical problems, or other security matters. When contract employees report to the institution but are unable to perform services due to such delays, the contractor will be paid for one (1) session. Normally, any time remaining for that day will be suspended and the contractor will be free to leave and the contractor will not be paid for remaining unperformed sessions. However, if the institution requests that the contractor remain at the institution for eventual performance, the contractor shall be paid for such sessions. In the event of any absence, the contractor shall provide replacement coverage. If the contractor fails to provide coverage, the Government may seek contractual remedy. Partial sessions are not authorized. METHOD OF PERFORMANCE: The Contractor has sole responsibility and discretion for choosing and implementing the manner and method of performing tasks outlined in this Solicitation as long as the requirements are met in accordance with the policies described above and in accordance with standards and methods generally accepted within the Contractor's own field of expertise. The services, although not directly supervised, shall be reviewed by institution medical staff to ensure contract compliance. Contract monitoring reports will be prepared by the COTR and maintained in the contract file. RESTRICTIONS ON CONTRACT WORKERS: The Contractor shall not supervise inmates, supervise Bureau of Prisons staff, participate in inmate accountability, requisition, sign for, or pick up supplies, formulate policy or procedures, or attend meetings, recalls or staff functions. INSTITUTION SECURITY: The contractor must agree to adhere to all regulations prescribed by FCI Elkton, for the safety, custody and conduct of inmate/patients. FCI Elkton shall be responsible for maintaining security of inmates. The safety and well being of other patients serviced by the contractor must remain a priority consideration at all times. The custody of Federal inmates remains the primary responsibility of the designated facility (i.e. FCI/FSL Elkton, OH). MISCELLANEOUS INFORMATION: In accordance with the Federal Bureau of Prisons Acquisition Policy, Part 37.103(e), the following is applicable to the work being performed in the contract: The service is a contractual arrangement and not a personnel appointment; payment is based on the provision of an end product or the accomplishment of a specific result; the services provided does not constitute an employer/employee relationship; and the contractor will not be subject to Government supervision, except for security related matters. However, contractor performance shall be closely monitored. Additionally, this position is a contract position only and does not provide fringe benefits such as Health Care, Retirement, Vacation or Sick Time. PRICING METHODOLOGY: The contractor shall provide non-personal services, upon request by the institution as detailed in this Solicitation. The Government reserves the right to award without discussion. Therefore, each initial quote should contain the quoter's best terms from a cost or price standpoint. Contract pricing shall be based on a rate per session and include all charges to the Government for performing the services required by this contract (e.g., bonding, overhead, profit, supervision, minimum wages, etc.). The quantity of services to be provided by the contractor is estimated below. These estimates are not a representation to a quoter or contractor that the estimated quantities will be required or ordered, or that conditions effecting requirements will remain stable or normal. Please note that there will be processing time required to enter the institution that should be reflected in the price quoted. Contract pricing shall include all charges to the Government for performing the services required by this solicitation/contract. Quoters are required to submit pricing for the base year and all option periods. Pursuant to FAR 17.203(b), the Government's evaluation shall be inclusive of options. Pursuant to FAR 17.203(d), the quoter may offer varying prices for options, depending on the quantities actually ordered and the dates when ordered. It is anticipated that a contract award resulting from this solicitation will be made approximately November 1, 2006. NON-PERSONAL SERVICES STATUS: Award of a contract will result in a contractual arrangement only and will not be construed as a personnel appointment with the Bureau of Prisons. The services will not constitute an employer/employee relationship. Payments to the Contractor will be based on the provision of an end product or the accomplishment of a specific task. Results to be obtained are within the Contractor's own unsupervised determination. The Contractor will not be subject to Government supervision but its efforts will be monitored for quality assurance. PERIOD OF PERFORMANCE: The anticipated period of performance of the resulting contract will be as follows: Base Year: Effective Date of Award through 12 months; Option Year 1: 13-24 months from Effective Date of Award; Option Year 2: 25-36 months from Effective Date of Award; Option Year 3: 37-48 months from Effective Date of Award; Option Year 4: 49-60 months from Effective Date of Award. The Contractor will commence performance of this contract no later than 30 calender days from the effective date of the award of this contract. In accordance with FAR 46.503 and 46.403(a)(6), the place of acceptance for services under this contract is at the destination. The Contractor will provide the contract services independent of Government supervision. INSTITUTION SECURITY/INVESTIGATIVE REQUIREMENTS: The Contractor and/or employees of the contractor entering the institution shall meet certain security requirements to receive an institutional pass as required by Bureau of Prisons Program Statement 3000.02 dated March 10, 1994. PRIVACY ACT NOTIFICATION: The contract will require contract staff to operate a system of records documentation that will require adherence to the Privacy Act of 1974. The system of records will be maintained in accordance with community standards for health service medical records. The contractor and/or staff are not authorized to disclose this or any information pertaining to an inmate to any non-BOP source. Any requests for such information shall be forwarded to the Health Services Administrator for proper disposition.
