Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation number RFQ-0620064 is issued as a Request For Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-12. The North American Industry Classification System (NAICS) code is 541611 with a small business size standard of $6.5 million. This procurement is not being conducted as a set-aside, and is available for full and open competition. The duration of the contract shall be one year from date of award and the contract will include four 1-year options to be exercised at the sole discretion of the Commodity Futures Trading Commission (CFTC) pursuant to FAR Clause 52.217-9, Option to Extend the Term of the Contract. As described in the Statement of Work attached to this synopsis/solicitation, the Contractor shall provide Employee Assistance Program services to CFTC employees and their family members. Any deliverables shall be shipped F.O.B. Destination. The provision at FAR 52.212-1, Instructions to Offerors-Commercial Items (Jun 2006) applies to this acquisition. The provision at FAR 52.212-2, Evaluation-Commercial Items (Jan 1999) is applicable. The specific evaluation criteria to be included in paragraph (a) of that provision are: 1) Technical Understanding and Approach; 2) Past Performance (not more than 3 references); 3) Key Personnel; and 4) Price. Each quoter shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (Jan 2006) with its quotation. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items (Sep 2005) applies to this acquisition. The clause at FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items (Aug 2006) applies to this acquisition. The clause at FAR 52.217-5, Evaluation of Options (Jul 1990) applies to this acquisition. The following FAR clauses are applicable to the acquisition: 52.217-9, Option to Extend the Term of the Contract (Mar 2000): (a) The Government may extend the term of this contract by written notice to the Contractor within the contract period of performance; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 60 months; 52.222-3, Convict Labor (Jun 2003); 52.222-19; Child Labor?Cooperation with Authorities and Remedies (Jan 2006); 52.222-21, Prohibition of Segregated Facilities (Feb 1999); 52.222-26, Equal Opportunity (Apr 2002); 52.222-35, Equal Opportunity for Special Disabled Veterans, Veterans of the Vietnam Era, and Other Eligible Veterans (Dec 2001); 52.222-36, Affirmative Action for Workers with Disabilities (Jun 1998); 52.222-37, Employment Reports on Special Disabled Veterans, Veterans of the Vietnam Era, and Other Eligible Veterans (Dec 2001); 52.223-9, Estimate of Percentage of Recovered Material Content for EPA-Designated Products; 52.225-13, Restrictions on Certain Foreign Purchases (Feb 2006); 52.232-34, Payment by Electronic Funds Transfer?Other than Central Contractor Registration (May 1999). This contract will be subject to the Department of Labor Service Contract Act Wage Determination No. 1994-2103, Revision No. 35. A copy of the Wage Determination may be obtained from the Contracting Officer. Quotations must include: 1) Technical Understanding and Approach: Discuss your understanding of the statement of work and technical approach for accomplishing this requirement, including a discussion of credential standards for counselors, resources available for offsite counseling sessions and capability of replacing key personnel (limit 6 pages); 2) Past Performance: Provide information on three (3) projects similar in scope, magnitude and complexity to this project that are currently being completed or have recently been completed, including point of contact name, phone number and email address; 3) Key Personnel: Provide a resume for the Account Manager proposed to perform under this order, including work and education history, that demonstrates the experience and qualifications necessary to successfully perform the work outlined herein, and supervisory reference information (including name, phone number and email) from three (3) most recent positions (limit 2 pages); 4) Price: Fixed rate per employee for base year and each option year (COMPLETE Attached schedule A); and 5) Completed copy of FAR 52.212-3, Offerors Representations and Certifications-Commercial items [this form is available on the Internet at http://www.acqnet.gov/far]. The Government anticipates awarding one purchase order resulting from this combined synopsis/solicitation to the responsible offeror whose quotation conforming to this synopsis/solicitation is determined to be the most advantageous to the Government, price and other factors considered. The Government will evaluate price by adding the base year price and the price for each option year to arrive at a total price for the requirement. All responsible sources may submit a quotation and will be considered by the agency. Quotations shall be sent to the attention of Sonda Owens, Contracting Officer, by email to sowens@cftc.gov. All quotations must be received not later than 4:30 PM on 20 September 2006. Questions regarding this RFQ should be directed to Sonda Owens, Contracting Officer via email not later than 3:00PM on 12 September 2006. Statement of Work - EMPLOYEE ASSISTANCE PROGRAM The Commodity Futures Trading Commission has a need for a Contractor to provide its employees and their family members with Employees Assistance Program services, for personal and/or work-related conflict and/or concerns. The CFTC has approximately 476 employees, located in five (5) offices throughout the United States (Headquarters office: Washington, DC; Regional offices: Chicago, IL; New York, NY; Minneapolis, MN; Kansas City, MO). At this time, the agency does not anticipate that the number of employees will change significantly over the duration of the contract; however, if the number does change by 10%, either party may request an equitable adjustment, pursuant to FAR clause 52.243-1, Changes ? Fixed Price (Aug 1997), Alternate III. The Contractor shall be responsible for providing counseling, educational information and referrals to products, services and programs to assist the employees and/or their families with a wide range of concerns, including emotional, behavioral, mental or personal concerns, as well as concerns related to alcohol or drug abuse, all of which may adversely impact the employee?s job performance, attendance, and or conduct. Referral services shall include referrals for child care, elder care, legal and financial services, with a minimum of the initial consultation being free of charge to the employee and or family. The Contractor shall assign a dedicated Account Manager to the CFTC who shall be responsible for coordinating on-site service requests, including counseling sessions; coordinating and assisting the CFTC with health and other EAP orientation sessions, promotions, and periodic (quarterly) on-site workshops designed to educate and inform employees about the program and its associated benefits; working directly with the CFTC Contracting Officer?s Technical Representative (COTR) to assist in promoting and periodically assessing employee satisfaction with the program and providing the CFTC COTR with reports of the satisfaction surveys; for providing the CFTC COTR with periodic newsletters for purposes of updating staff about the program services; providing other reports as requested; and otherwise ensuring that the Contractor complies with the terms and conditions defined herein. The Account Manager shall be considered key personnel and any replacement of key personnel must be supported by a resume, submitted to CFTC for approval, prior to the date the individual starts work under this order. The resume must demonstrate that the replacement is qualified to perform the work for which he or she is proposed. Any replacement is subject to disapproval by the CFTC Contracting Officer. In the event a replacement candidate is disapproved, the Contractor shall take immediate action to locate and propose another candidate. The Contractor shall provide 24-hour, 7 days a week, access to assistance via a toll free number. Services shall include unlimited telephonic consultation for employee managers, employees and their family members and staff members of the CFTC Office of Human Resources Management. In the event a crisis situation occurs at any of the CFTC locations, the Contractor shall conduct a critical incident debriefing within 24 hours of the request. If/when special situations warrant (i.e.; death, bombing, or other traumatic situations) debriefings shall be conducted within 12 hours of the request, unless otherwise specified by authorized CFTC staff. The Contractor shall be responsible for providing direct, short-term, in-person counseling, a minimum of six (6) counseling sessions per problem, per calendar year, as requested. Counseling sessions shall be held at contractor furnished off-site locations, and these offsite locations shall be within a 30 mile radius of the CFTC office location in which the employee works and/or the home location in which the employee and/or family member lives. Counselors must have a minimum of three (3) years of Employee Assistance Program counseling experience and shall be licensed professional counselors and/or social workers in the state in which they are providing services. The Contractor shall maintain confidentiality of all EAP counseling records and information from employee visits shall be kept in a confidential manner, in accordance with privacy act laws, regulations, and applicable Office of Personnel Management policies.
