Description
COMBINED SYNOPSIS/SOLICITATION This is a combined synopsis/solicitation for services non-personal issued by the 55th Contracting Office, Offutt AFB, Nebraska. This document was prepared in accordance with the format found in the Federal Acquisition Regulations Document (FAR) subpart 12.6, as supplemented with additional information included in this notice. This electronic announcement is the only solicitation being issued. A WRITTEN SOLICITATION WILL NOT BE ISSUED. The solicitation document and incorporated provisions/clauses are those in effect through Federal Acquisition Circular (FAC) 97-13. The government intends to issue a contract for the following service - Clinical Reference Laboratory Services. The Contractor shall provide clinical reference laboratory services to Ehrling Bergquist Clinic (EBC) Laboratory at Offutt Air Force Base, Nebraska. The Contractor shall provide all required facilities, personnel, services, transportation, materials, and supplies required. Use of this contract will be limited to those at the Ehrling Bergquist Clinic (EBC) laboratory. All tests shall be executed at the contractor?s premises, or within the Contractor?s network of laboratories (affiliate or qualified subcontractor). The solicitation is unrestricted and has an anticipated delivery date of 01 October 2006 with four option years to end 30 September 2011. The North American Industry Classification System (NAICS) code is 621511 and the small business size standard is $12.5 million (See attached Performance Work Statement). Funds are not presently available for this contract. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer. All firms or individuals responding must be registered with the Central Contractor Registration (CCR). All services to be performed should be included in the proposal. All proposals shall be received by 19 September 2006, 4:00 pm (CST) faxed to Mr. Charles S. Crivellaro at (402) 294-7280 or e-mailed to charles.crivellaro@offutt.af.mil. The following clauses and provisions will apply to this acquisition. FAR 52.212-1, Instructions to Offerors. 52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) technical capability of the service offered, in accordance with the PWS, to meet the Government requirement (ii) price (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. (End of clause) Offerors shall include a complete copy of the provision at FAR 52.212-3, Offerors Representations and Certifications-Commercial Items, with the offer. Crivellaro at (402) 294-7280 or e-mailed to charles.crivellaro@offutt.af.mil. 05 Sep 2006/ QUESTIONS AND ANSWERS QUESTION: Because the amount is predicated on the number of each test ordered, what figures are you looking for in the amount fields and is there a listing of the volume of each test? ANSWER: The government needs to have ALL the tests on the price list available. And there is no guessing on how many, because government never knows when they?ll be needed. Histology also has to be included. QUESTION: There are no volumes specified for any of the tests. Also the government has some tests listed that would require the provision of a pathologist and equipment on the hospitals site to actually perform, i.e. Frozen Section (two items) and or Consultation and Fine Needle Aspirate, Immediate Interpretation. And that these are procedures performed in a hospital requiring a pathologist to be in attendance. ANSWER: it is impossible for the government to quantify how many samples will be sent to a reference laboratory because the need for testing is based on several factors outside the control of the laboratory. For example, if one of our doctors needs a test results STAT and we routinely send it to a military reference laboratory we will send to a local reference laboratory instead for analysis. The government requires a cost for each test and utilize the services on an as needed basis
