- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The reference number is FM442762050585; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12 and Defense DCN 20060814. This acquisition will be a 100% small business set-aside; the North American Industry Classification System (NAICS) code is 334510. The business size standard is 500 Employees. The Federal Supply Class (FSC) is 6515. The Standard Industrial Classification (SIC) is 3845. This request for quotation consists of the following items (Note: All products are KayPentax part numbers. Equal products will be considered. Technical data shall be provided for all ?equal? products for Government review.): Item 0001: 1ea Digital Video Capture module, part # 9200C. Digital Video recording system with all hardware and software for recording examinations and interfacing to camera, light source, and printer. Includes computer system, added special boards for video acquisition camera control, light source interface. System also includes custom software. Item 0002 : 1ea 3-CCD camera, part # 9212. Camera with three ? inch CCD sensors with DSP, S-video, variable AGC, 700 line resolution, computer controllableand c-mount. NTSC format. Item 0003: 1ea 22-44mm Zoom Lens Coupler, part # 9118. Adjustable focal length lens with eye piece adapter. Suitable for all scopes. Item 0004: 1ea Color printer, part # 9264C. Ink-Jet printer (digital) which uses 8-1/2 ?x 11? paper and 4? x 6? gloss photography paper. Item 0005: 1ea EPK-1000, part # KVM66116. Video Processor for all K series video scopes. Produces full screen images with optimized color and illumination adjustments. Includes 100 watt xenon light source, keyboard, water bottle, and air pump. Item 0006 : 1ea Nasolaryngoscope 70K series, part # VNL-1170K. ENT video flexible endoscope with color CCD chip. Distal tip Diameter: 4.1mm, Insertion tube diameter: 3mm,working length: 300mm, angulation up/down: 130/130, angle of view: 85 degrees. Requires EPK-1000 video processor. Item 0007: 1ea set up and training at Travis AFB. The following provisions and/or clauses apply to this acquisition. FAR 52.212-3, Offeror Representations and Certifications-Commercial Items. An offeror shall complete only paragraph (j) of this provision if the offeror has completed the annual representations and certificates electronically at http://orca.bpn.gov . If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (b) through (i) of this provision. FAR 52.212-3 shall be completed as noted above and submitted with the offer. Offerors must comply with all instructions contained in FAR 52.212-1 Instructions to Offerors - Commercial.
