- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation will not be issued. This requirement is being issued as a Request For Quotation. Reference Purchase Request number F2V4FM6187A600 on all documentaion to include the Quotation. This notice and the incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-09. 1. Description of Services. CLIN 0001 The contractor shall furnish all personnel, supplies, and equipment necessary to fabricate orthotic prescriptions. Materials should be lightweight, durable, and hypoallergenic; that are patient-friendly and can be wiped clean with a damp, soapy cloth, and can be regenerated with a warm-air flow. Billing to be accomplished on a monthly bases. This acquisition is 100% set-aside for small business, the North American Industry Classification System code (NAICS) is 339113, the size standard is 500 employees. Proposals must list the DUNS number, CAGE code, and Federal TIN. If you need to obtain or renew a DUNS number or CAGE code, please visit www.ccr.gov. All contractors doing business with the Federal Government must be registered in CCR. DELIVERY ADDRESS: 8415 Bayshore Blvd, MacDill AFB, FL 33621-1607. PERIOD OF PERFORMANCE: Performance will start on 1 Oct 2006 thru 30 Sep 2007. Numbered Note: 1 applies PROVISIONS/CLAUSES: The following FAR and DFARS provisions and clauses apply: 52.212-1, Instruction to Offerors-Commercial. In accordance with 52.212-2, Evaluation-Commercial Items, offers will be evaluated based on price, technical acceptability, and past performance records.
