Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation is a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-12 and DFARs Change Notice (DCN) 20060814. This acquisition is a small business set-aside. NAICS is 488991 and FSC is V003. Solicitation is for commercial purchase of: The contractor shall provide the following based on the information stated: The 412th Test Engineering Group, Edwards Air Force Base, CA is looking to procure the Relocation Service as described below in Statement of Work. STATEMENT OF WORK FOR RELOCATION SERVICE 412th Test Engineering Group, Edwards AFB CA 11 August 2006 1.0 DESCRIPTION OF SERVICES 1.1. Scope of Work. Contractor Responsibility: The work covered by, but not limited to, specifications consisting of furnishing all supervision, labor, equipment, and material (to include furniture blankets) to perform all operations for movement of office equipment (computer, printers etc), supplies, file cabinets, and miscellaneous office items in relocating approximately 125 personnel to two different locations. Computer moves include CPUs, monitors, keyboards, and assorted wiring. Personnel will unplug wiring prior to being moved. 2.0 SERVICE DELIVERY SUMMARY 2.1 Vendor will provide the following items in approximately 2 weeks in advance of scheduled move for personnel to prepare items for moving: 1) Boxes ? 10 per person (1200 boxes), 5 cubic Ft in size 2) Tape ? 100 rolls 3) Labels ? to mark boxes 2.2 Vendor will disassemble any 2X2 storage cabinets and reassemble at delivery point. 2.3 Movement will be completed in two stages: First stage will consist of movement of subject items from Building 1400 (AFFTC/XPD and Tybrin offices) to Bldg 2750 and Bldg 1864. Second stage (approximately 10 working days later), items from Bldg 1609 will be relocated to Bldg 1400. 3.0 GOVERNMENT FURNISHED PROPERTY AND SERVICES 3.1 Government Furnished Equipment or Facilities. The Contractor will furnish all equipment necessary to transport items between locations. 4.0 GENERAL INFORMATION 4.1 Work Hours: Work hours for the relocation of items will be Monday through Friday, 7:30am to 4:30 pm. No work shall commence without prior coordination with the facility manager, Regina Jimenez (661) 277-0728, and the project manager, Kenita Bushee, 277-7189. 4.2 Base Access: Proof of vehicle registration, driver?s license, and vehicle insurance must be made available prior to issuance of base permits. The contractor shall obtain base access for his personnel, subcontractors, vehicles and equipment by submitting the following information to the contracting office or facility manager or project manager within five (5) working days prior to commending work. NAME OF INDIVIDUAL SOCIAL SECURITY NUMBER DRIVERS LICENCE NUMBER VEHICLE LICENSE NUMBER The offeror shall include descriptive literature such as specifications, brochures or other manufacturing information that fully describes the item if not the suggested manufacturer and part number. Delivery of boxes must available immediately upon receipt of purchase order and FOB Destination. Place of delivery, performance, and acceptance is FOB Destination, Edwards AFB, CA 93524.
